Objective: To explore the clinical efficacy of Qilian Sijunzi Decoction in the treatment of peripheral neuropathy in elderly patients with type 2 diabetes, and to explore its mechanism of action,in order to provide new ideas and methods for the prevention and treatment of diabetic peripheral neuropathy with integrated traditional Chinese and Western medicine. Methods: According to the inclusion and exclusion criteria, patients with type 2 diabetes peripheral neuropathy(spleen deficiency and blood stasis syndrome)aged 60 in the Outpatient Department of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine and the Department of Geriatrics were selected from March 2017 to March 18,2017.example.The patients were divided into treatment group and control group by random single-blind method. The two groups were given basic treatment(1.diabetes education 2.diet control 3.exercise therapy 4.drug treatment 5.self-glycemic monitoring)and active control of blood pressure Blood lipids, control group routinely control blood sugar, blood pressure, blood lipids, oral nutritional neuropharmaceutical methylcobalamin, the treatment group was treated with traditional Chinese medicine Qilian Sijunzi Decoction at the same time as conventional treatment, the two groups were treated for 4 weeks as a course of treatment,3 treatments The course of treatment, the main baseline data, clinical efficacy and the TCM syndrome scores of the two groups before and after treatment, the clinical score of peripheral neuropathy, blood glucose index, laboratory indicators(iron, serum ferritin, calcium, phosphorus)were observed.,vitamin D,vitamin B12),neurophysiological examination-nerve conduction velocity(NCV)(median nerve, ulnar nerve, common peroneal nerve, radial nerve, sural nerve),using SPSS25.0 statistical software for data analysis, evaluation Clinical efficacy and safety. Results: 1.General information: There was no significant difference in age, gender and disease duration between the two groups(P>0.05),which was comparable.2.Total effective rate of TCM syndrome: The total effective rate of the treatment group was 86.67%,which was significantly higher than that of the control group(63.33%),and the difference was statistically significant(P<0.05).3.Compared with before treatment, the TCM syndrome scores of the two groups were significantly decreased after treatment(P<0.05),and the treatment group was significantly lower than the control group, the difference was statistically significant(P<0.05).4.Compared with before treatment, the levels of fasting blood glucose(FPG),postprandial 2 h blood glucose(2 h PG)and glycosylated hemoglobin(Hb A1c)were significantly decreased after treatment(P<0.05),and 2h PG and Hb A1 c were significantly lower in the treatment group. In the control group, the difference was statistically significant(P < 0.05).5.Compared with before treatment, serum ferritin levels were significantly lower in the two groups after treatment, the difference was statistically significant(P<0.05),and the treatment group was significantly lower than the control group, the difference was statistically significant(P<0.05).6.Compared with before treatment, the trace elements(iron, calcium, phosphorus)in the two groups were significantly increased after treatment, the difference was statistically significant(P<0.05),and the treatment group was significantly higher than the control group, the difference was statistically significant.(P<0.05).7.Compared with before treatment, vitamin D and vitamin B12 were significantly increased in the two groups after treatment, the difference was statistically significant(P<0.05),and the treatment group was significantly higher than the control group, the difference was statistically significant.(P<0.05).8.Compared with before treatment, the nerve conduction velocity of the two groups was significantly increased after treatment, the difference was statistically significant(P<0.01 or P<0.05),and the treatment group was significantly higher than the control group, the difference was statistically significant(P <0.05).9.Compared with before treatment, the TCSS scores of the two groups were significantly lowered, the difference was statistically significant(P<0.05),and the treatment group was significantly lower than the control group, the difference was statistically significant(P<0.05).10.There were no adverse reactions in the clinical observation of the two groups of patients, indicating that the two groups of drugs were safe and reliable in clinical application. Conclusion: Qilian Sijunzi Decoction can effectively reduce the blood sugar level of elderly patients with type 2 diabetes peripheral neuropathy, regulate its metabolism of iron, serum ferritin, calcium, phosphorus, vitamin D,vitamin B12,etc.,improve the nerve conduction velocity of patients and relieve the peripheral nerves. The clinical symptoms of the lesions are significant and have good safety.
