Clinical Observation On The Treatment Of Chronic Kidney Disease 3-5 Period By The Method Of Invigorating Spleen,Tonifying Kidney And Removing Damp-turbidity,Dredging Collaterals

Objective: The purpose of this study is to observe the clinical efficacy and safety of the method of invigorating spleen,tonifying kidney and removing damp-turbidity,dredging collaterals treating Chronic Kidney Disease 3-5 period patients with deficiency of spleen and kidney,obstruction of damp-turbidity and blood stasis syndrome,and to explore its effects on renal function and microinflammatory state,so as to provide new methods and new ideas for CKD 3-5 period.Methods: Sixty patients who met the test criteria were randomly divided into control group and treatment group with 30 cases in each group.On the basis of routine treatment,the control group was treated with Niaoduqing Granule,while the treatment group was treated with the prescriptions of invigorating spleen,tonifying kidney and removing damp-turbidity,dredging collaterals on the basis of the control group.Both groups were treated for 8 weeks.Then to observe the changes of total effect of disease,TCM syndromes efficacy,renal function index(Scr,BUN,e GFR),microinflammatory state index(hs-CRP,TNF-α),24-hour urinary protein quantification(24h UPQ),serum albumin(ALB),hemoglobin(Hb),serum cholesterol(TC),triglyceride(TG)and so on before and after treatment,and though statistical analysis to evaluate efficacy and safety.Results:(1)Total effect of disease: The total effective rates of treatment group and control group were 93.33% and 80.00%,respectively.There were statistical differences between the two groups(P<0.05),and the treatment group was better than the control group.(2)Curative effect of TCM syndromes:The total effective rates of the treatmentgroup and the control group were 93.33% and 76.67%,respectively.There were statistical differences between the two groups(P<0.05),and the treatment group was better than the control group.(3)Renal function indicators:There were statistical differences in Scr,BUN and e GFR between the two groups before and after treatment(P<0.05).After treatment,there were statistical differences between Scr and e GFR(P<0.05),and the treatment group was better than the control group.(4)Indicators of microinflammatory state: There were statistical differences in hs-CRP and TNF-α between the two groups before and after treatment(P<0.05).After treatment,there were statistical differences between hs-CRP and TNF-α(P<0.05),and the treatment group was better than the control group.(5)24h UPQ,ALB indicators:Compared with pre-treatment,24 h UPQ in two groups and ALB in treatment group had statistical difference after treatment(P<0.05),but there was no statistical difference in ALB in control group(P>0.05).After treatment,there was statistical difference in ALB between the two groups(P<0.05),and the treatment group was better than the control group.(6)Indicators of Hb,TC and TG: Compared with pre-treatment,there were statistical difference in Hb,TG between the two groups and TC in the treatment group after treatment(P<0.05),but there was no statistical difference in TC in the control group(P>0.05).There was statistical difference in Hb,TC between the two groups after treatment(P<0.05),and the treatment group was better than the control group.Conclusion:The method of invigorating spleen,tonifying kidney and removing damp-turbidity,dredging collaterals has good clinical effect in the treatment of CKD3-5period,and have no obvious adverse reaction.It can significantly improve clinical symptoms and signs,improve renal function and delay the progress of CKD.The mechanism may be related to the improvement of microinflammatory state and the correction of complications such as renal anemia,hypoproteinemia and dyslipidemia and so on.It is worth further study and popularization.
