The Effect Of Different Time Moderate Intensity Exercise On Blood Pressure In Male Patients With Stage 1 Hypertension And Potential Mechanism

Objectives: The purpose of this study was to observe the effect(during and after interventions)of 20-minute and 40-minute(defined as E20 and E40 group)moderate intensity(40-50% HRR)exercise on men with grade 1 hypertension who had not taken hypotensor,and tried to explore the mechanism of acute exercise on blood pressure,so as to provide a theoretical basis for the formulation of individualized exercise prescription for patients with grade 1 hypertension.Method: Recruiting 18 men(34.8±3.5 years)with grade 1 hypertension who had not taken hypotensor.They were monitored by ambulatory blood pressure,morphology,physical activity and exercise interventions.Blood pressure and heart rate in quiet,exercise(every 5 min),and after exercise(3th min,6th min)were monitored during the intervention.The anterior elbow vein blood was extracted immediately before and after exercise to detect adrenaline,angiotensin II,nitric oxide,endothelin-1 and endothelial micro-particles.Results:1.SBP in two groups increased significantly(38.22±10.40 mm Hg vs33.47±12.15 mm Hg),E20 group was stable,E40 group 25 th to 35 th min decreased 8mm Hg,and then stabilized(P<0.01).There was no obvious change of DBP in the two groups.Two groups of HR during exercise significantly increased(53.88±9.59 bmp vs 51.42±12.66 bmp),E20 group HR increased slightly from 15 th to 20 th min,E40 group increased gradually from 25 th min until the end(P<0.01).2.Recovery period:6th min after exercise SBP in E20 group was no difference with the resting,6th min after exercise SBP in E40 group decreased by 13 mm Hg(P <0.01).DBP in E20 group was the same as resting at 3th min after exercise,and DBP decreased 4 mm Hg at 6th min after exercise(P <0.05),while DBP decreased 2 mm Hg at 3th min after exercise in E40 group(P <0.05).3th min and 6th min after exercise,the HR of E20 group decreased by about 29 bmp and 41 bmp,respectively(both P <0.01).3th min and6 th min after exercise,the HR of E40 group decreased by about 32 bmp and 35 bmp,respectively(both P <0.01).3.The levels of plasma A,AngⅡ and NO in both groups increased significantly after exercise compared with those before exercise,and the increases in E40 group were greater than those in E20 group(all P<0.01).The plasma ET-1 and EMPs concentration in two groups decreased significantly compared with those before exercise,and the decline in E40 group was larger than that in E20 group(all P<0.05).Conclusion:1.There are no any abnormal heart rate or blood pressure response in20-minute and 40-minute moderate intensity exercise for the early hypertensive men who have not taken hypotensor and overweight or obses.2.20-minute and 40-minute moderate intensity exercise all can reduce the rest blood pressure and make the hypertensive men gain the related benefit of blood pressure control,and the effect of40-minute moderate intensity exercise is better than that of 20-minute moderate intensity exercise.3.The possible reason for the increase of blood pressure during exercise is the increase of SNS and RAS activity,The possible mechanism of the decrease of blood pressure after exercise is the increase of NO and ET-1concentration,and the improvement of endothelial function.
