Objective In the hospital,through the establishment of appropriate the hospital of traditional Chinese medicine prescription comment mode,sound systematization and standardization and continuous improvement of the traditional Chinese medicine prescription review system,and ultimately to improve the level of rational use of drugs in the hospital,to ensure the safety of drug use in the clinic.MethodsThrough the establishment of related systems and organizations,establish various medication management system,the establishment of the hospital of traditional Chinese medicine prescription review criteria and clinical pharmacist system,established the Chinese medicine prescription review system,clear classification and content of prescription comment.Time intervention for from January to December 2014.The research status of the traditional Chinese medicine prescription comment under the mode of the assessment.1 Prescription of Chinese Herbal PiecesChinese herbal medicine prescriptions from January to December 2014 to extraction,using Excel software to establish database,carries on the statistical analysis of pumping the prescription of Chinese herbal medicine from the drug dose,special usage,incompatibility,medication,differentiation of symptoms and signs,seven aspects of drug taste number and prescribed price comments.2 prescription reviews of Chinese MedicineChinese herbal medicine prescriptions from January to December 2014 to extraction,establish a database using Excel software and statistical analysis.To the pumping of proprietary Chinese medicine prescription from from special outpatient cases exceeding the prescribed amount,incompatibility,inappropriate amount of suspected repeated drug use,combined medication is not appropriate,no indications of medication,diagnosis of incomplete or not standardized,selection of drugs is not suitable,usage is not appropriate,drug use in special populations are not suitable 10 aspects of comments.Results Our hospital 2014 prescriptions of traditional Chinese medicine from January to December unqualified rate from about 25.00%drop to 8%.4800 prescriptions of 10 kinds of following prescription accounted for the proportion of total prescription was 10.43%,10-17 flavor prescription drugs accounted for total prescription proportion was 48.75%,18-24 flavor prescription accounted for total prescription proportion was 29.79%,more than 25 kinds of prescription accounted for total proportion of prescription prescription of 11.02%.Each agent price of 40 yuan,following the prescriptions accounted for the proportion of the total prescription is 74.04%,each agent price 40-50 yuan prescription accounted for total prescription proportion of 19.67%,each agent price 50 yuan above prescription accounted for total prescription proportion was 6.29%.Our hospital in 2014 unreasonable prescription medicine 1325 Zhang,accounted for the total number of prescription 22.08%,which does not regulate prescription 402 pieces,accounting for unreasonable prescription rates 30.34%,inappropriate medication prescriptions of 876,accounting for unreasonable prescription rate of 66.11%and 47 abnormal prescriptions,accounting for unreasonable prescription rate of 3.55%.In 2014 monthly unqualified number of prescriptions in monthly decline.Conclusion 1 the establishment of traditional Chinese medicine prescription evaluation model in line with the basic situation of our hospital;2.Under the model established by the comment and intervention,our hospital both Yinpian prescription,medicine prescription standardization of clinical medication and rationality have the significantly improved;3 irrational drug use phenomenon still exists,pieces of management indicators are still not up to standard,the need for continuous intervention,continuous improvement,clinical rational drug use is the eternal theme.
