Objective: To explore the clinical value of serum YKL-40 in chronic airway inflammatory diseases by comparing the expression level of serum YKL-40 in asthma,COPD and ACO patients and the relationship with clinical features.Methods: A total of eighty-eight patients in the respiratory department of the Second Hospital of Hebei Medical University from January 2020 to December 2020 were selected as the research subjects.According to their conditions,the patients were divided into group of COPD(n = 25),asthma(n =34)and ACO(n = 29).The demographic characteristics and smoking status of the patients were collected,and the pulmonary function was examined.The results of FEV1%pred,FEV1/FVC and FENO were recorded,and the level of serum YKL-40 was detected at the same time.Spearman’rank correlation test was used to analyze the correlation between serum YKL-40 level and demographic characteristics,pulmonary function and FENO.The receiver working characteristic curve was used to evaluate the clinical value of YKL-40 in chronic airway inflammatory diseases.Results: 1.The age and smoking index of ACO group and COPD group were higher than those of asthma group,while FEV1%pred and FEV1/FVC were worse.The level of FENO in ACO group and asthma group was higher than that in COPD group.The difference was statistically significant(P<0.05).2.The serum YKL-40 levels in ACO group and COPD group were(168.24±0.89)ng/m L and(181.33±80.81)ng/m L,respectively,which were higher than those in asthma group(124.9 ± 53.88)ng/m L,but there was no significant difference between ACO and COPD group(P=0.501>0.05).3.The level of serum YKL-40 was negatively correlated with pulmonary function parameters FEV1%pred(r=-0.211,P<0.05)and FEV1/FVC(r=-0.293,P<0.05).However,there was no significant correlation between serum YKL-40 level and FENO(r=-0.104,P=0.336>0.05).4.The AUC of serum YKL-40 to distinguish ACO from asthma and COPD was 0.595(P=0.149>0.05).Conclusions: Serum YKL-40 is significantly different between ACO and asthma patients,asthma and COPD patients,and negatively correlated with pulmonary function parameters.As a biomarker of airway inflammation and airway remodeling,serum YKL-40 has certain value for chronic airway inflammatory diseases.
