Objective To explore the distribution of the frailty syndrome among the elderly in the community of Inner Mongolia Autonomous Region,and to understand the relevant factors affecting and improving the frailty syndrome through follow-up visits,so as to provide beneficial suggestions for the prevention or intervention of the frailty syndrome among the elderly in the community in the future.Methods For cohort study,this study selected ho xilingol league area cover long-term living in Inner Mongolia autonomous region administration building a community of people,six to nine months in 2019 to participate in the examination,and conform to the standard of more than 65 years old and 736 old people community as the research object,into groups according to Fried scale assessment weak state,the population is divided into weak group,the early stage of the weak group and healthy group,for the weak crowd followed up for 1 year,as of September 1,2020:After follow-up,the frailty group was divided into three groups:frailty improvement group,frailty maintenance group and frailty progress group.The influential factors of frailty change between the three groups at baseline and the end of follow-up were compared to explore the related factors affecting and improving the frailty syndrome of the elderly.Results(1)Among 736 community elderly≥65 years old,139 cases(18.9%)were frailty,and 395 cases(53.7%)were frailty in early stage.Follow-up after 1 year(10 patients lost to follow-up)showed that 20 cases(15.5%)had improved their frailty,68 cases(52.7%)had maintained their frailty,and 41 cases(31.8%)had advanced frailty.(2)Univariate analysis results on the distribution of initial frailty states showed that:Different gender(χ~2=39.770,P<0.001),age(χ~2=69.381,P<0.001),education level(χ~2=19.967,P<0.001),disease type(χ~2=41.700,P<0.001),drug use type(χ~2=102.084,P<0.001),family income(χ~2=16.580,P=0.002),smoking status(χ~2=223.805,P<0.001),BMI(χ~2=15.302,P=0.018),ADL score(χ~2=165.221,P<0.001)and SARC-F total score(F=176.523,P<0.001)were different in the frailty group.Univariate analysis of the results of fission status change group after follow-up showed that there were differences in gender(χ~2=8.225,P=0.016),type of disease(χ~2=25.832,P<0.001),type of drugs taken(χ~2=19.333,P=0.001),smoking status(χ~2=24.967,P=0.001),ADL score(χ~2=13.206,P=0.001)among different groups.(3)Multivariate Logistic regression analysis in the initial fission state showed that women(OR=2.972,95%CI=1.391~6.534,P=0.005),aged≥80 years old group(OR=2.747,95%CI=1.128~6.685,P=0.026),disease type≥3(OR=8.375,95%CI=3.241~21.64,P<0.001),disease type 1-2(OR=3.894,95%CI=1.686~8.994,P=0.001),taking drugs≥3(OR=2.641,95%CI=1.083~6.44,P=0.033),smoking(OR=7.48,95%CI=2.987~18.732,P<0.001),smoking cessation(OR=6.127,95%CI:2.766~13.571,P<0.001),abnormal ADL score(OR=22.933,95%CI=8.559~61.442,P<0.001)and high SARC-F score(OR=2.848,95%CI=2.144~3.782,P<0.001)were independent risk factors for frailty distribution,while family income≥20,000(OR=0.387,95%CI=0.156~0.958,P=0.040)were protective factors.Bariate Logistic regression analysis was performed in the fission progression group,and the results showed that smoking(OR=43.418,95%CI=8.771~214.930,P<0.001),smoking cessation(OR=3.868,95%CI=1.030~14.531,P=0.045),taking more than OR equal to 3 kinds of drugs(OR=14.644,95%CI=3.651~58.741,P<0.001),abnormal ADL(OR=3.137,95%CI=1.047~9.406,P=0.041)were the risk factors for frailty progression.Conclusion Elderly asthenia syndrome in the elderly community prevalence is not low,at the same time it is influenced by many factors,the community elderly asthenia syndrome related problems should cause enough attention,due to weak state is a dynamic variable,early detection can influence and change the process of weak factors,for the old patients with weak health outcomes is very important.
