Abstract/Summary:
Objective:This study analyzed the related factors of hypoglycemia in hospitalized patients with type 2 diabetes mellitus(T2DM),and further explore the related factors of elevated cardiac troponin I(cTnI)after hypoglycemia,so as to provide a new idea for the prevention and treatment of hypoglycemia in T2DM patients,make people pay more attention to myocardial damage caused by hypoglycemia.Methods:A total of 903 T2DM patients who were hospitalized in the Department of endocrinology of Northern Jiangsu Province Hospital from June 2019 to June 2020 and met the inclusion criteria were selected as the research objects.Among them,276 patients had hypoglycemia attack and 627 patients did not have hypoglycemia during hospitalization.The related influencing factors of hypoglycemia attack were analyzed.In addition,the cTnI of 276 subjects in hypoglycemia group was monitored.According to the level of cTnI at admission,the cTnI after hypoglycemia was compared and divided into normal cTnI group(n=225)and elevated cTnI group(n=51).The related influencing factors of elevated cTnI after hypoglycemia were analyzed.Result:1.The hypoglycemia group and non-hypoglycemia group were compared1.1 A total of 903 patients with T2DM were included in this study.There were 627 patients in the non-hypoglycemic group,accounting for 69.44%,and 276 patients in the hypoglycemic group,accounting for 30.56%.The general clinical data of the two groups were compared,including age,course of disease,body mass index,glycosylated hemoglobin,hemoglobin,white blood cells,serum albumin,triglyceride,total cholesterol,serum creatinine,glomerular clearance rate,maximum fluctuation range of blood glucose,diabetic kidney disease,diabetic retinopathy and diabetic peripheral neuropathy the difference was statistically significant(P<0.05).1.2 Taking hypoglycemia as the dependent variable and the indexes with statistical difference as the independent variable,binary logistic regression analysis was carried out.The results showed that triglyceride[OR(95%CI):1.021(1.000,1.042),(P<0.05)]and maximum blood glucose fluctuation[OR(95%CI):1.074(1.042,1.107),(P<0.05)],Body mass index[OR(95%CI):0.889(0.842,0.938),(P<0.05)],glycosylated hemoglobin[OR(95%CI):0.832(0.757,0.914),(P<0.05)],hemoglobin[OR(95%CI):0.983(0.973,0.992),(P<0.05],serum albumin[OR(95%CI):0.861(0.826,0.896),(P<0.05)]were protective factors of hypoglycemia in hospitalized T2DM patients.2.Comparison of cTnI elevated group and cTnI normal group in patients with hypoglycemia2.1 A total of 276 patients were included in the hypoglycemia group.Among them,225 patients(81.52%)had no change in cTnI test results after hypoglycemia,and 51 patients(18.48%)had elevated cTnI test results.The general clinical data of the two groups were compared.There were statistical differences in body mass index,history of coronary heart disease and diabetic nephropathy between the two groups(P<0.05).2.2 Binary logistic regression analysis was conducted with the increase of cTnI as the dependent variable and the indexes with statistical difference as the independent variable.The statistical results showed that body mass index[OR(95%CI):1.111(1.009,1.224),(P<0.05)],history of coronary heart disease[OR(95%CI):3.891(1.947,7.777),(P<0.05)],diabetic kidney disease[OR(95%CI):7.150(2.871,17.808),(P<0.05)]were risk factors for elevated cTnI in hypoglycemic patients.Conclusion1.Triglyceride and largest amplitude of glycemic excursion were risk factors of hypoglycemia in hospitalized T2DM patients;body mass index,glycosylated hemoglobin,hemoglobin,serum albumin and triglyceride were protective factors of hypoglycemia in hospitalized T2DM patients.2.Body mass index,history of coronary heart disease and diabetic nephropathy are the risk factors of elevated cTnI in patients with hypoglycemia.
