| Objective:(1)To observe the short-term effects of 3D printing-assisted hip revision;(2)To study the advantages of hip revision surgery with the aid of 3D printing technology compared to traditional technology.Methods 38 patients were enrolled,who were underwent hip revision from January 2018 to June 2020 in the Department of Joint Surgery of Yiyang Central Hospital.They are divided into two group,namely the 3D group(n = 19)and the control group(n = 19)according to the method of acetabular reconstruction.Patients in the control group used traditional surgery for hip revision and patients in the 3D group used 3D printing technology(preoperative measurement and evaluation,and using 3D printing titanium trabecular metal with or without 3D printed titanium augment during the operation)to assist hip revision.Observe hip function Harris score,VAS pain score,rotation center position changes(rotation center height and rotation center horizontal position)and other indicators of the 3D group before and after surgery.Analyze the differences of the indicators before and after surgery.When P< 0.05,it indicates that the difference of the indicator has statistical significance.Observe the operation time,blood loss,blood transfusion,acetabular prosthesis external abduction angle,rotation center position and other indicators of patients between the two groups.Analyze the differences of the indicators between the two groups.When P< 0.05,it indicates that the difference of the indicator between the two groups has statistical significance.Results Two groups patients were followded up on time.The follow-up time was 12 to 30 months,with an average of 14.28 months.(1)Comparing the preoperative and postoperative of 3D group,The last Harris score of the 3D group was(82.11 ± 5.69)points after surgery,which was significantly better than that before surgery(41.89 ± 8.93)points(P = 0.000).The final VAS score after operation was(1.21 ± 0.92)points which was better than that before surgery(5.63 ± 1.30)points(P =0.000).The postoperative rotation center position was significantly improved compared to preoperatively.The postoperative rotation center height was(26.92 ± 4.29)mm better than the preoperative rotation center height(34.45 ± 12.13)mm,and the difference was statistically significant(P = 0.001);The horizontal position of the posterior rotation center was(35.74 ± 5.83)mm better than the horizontal position of the preoperative rotation center(39.87 ± 8.59)mm(P = 0.012).Comparing the two groups,the operation time of the 3D group was(139.47 ± 44.16)min shorter than that of the control group(167.37 ± 40.12)min(P = 0.049).The intraoperative blood loss was(542.11 ± 234.08)m L of the 3D group which was less than that of the control group(831.58 ± 264.69)m L(P =0.001);the last Harris score of the 3D group was(82.11 ± 5.69)points after surgery which was better than the last Harris score of the control group(75.89 ± 7.30)(P = 0.006).The last postoperative VAS score of the3 D group was(1.21 ± 0.92)points which was better than that of the control group(2.26 ± 0.73)points(P = 0.000).The postoperative rotation center height of the 3D group was(26.92 ± 4.29)mm which was better than that of the control group(30.13 ± 5.17)mm(P = 0.045).Conclusion The 3D printing-assisted hip revision surgery can significantly relieve pain,improve hip joint function,and restore hip joint biomechanics.Compared with traditional surgery,3D printing technology assists hip joint revision,which has shorter operation time,less trauma,better functional recovery,and more advantages. |
