Objective:Three-dimensional reconstruction and print technology has a wide range of applications in the healthcare industry because of its advantages of accurate organization display and making complex entity shapes.During the operation of the Department of thoracic surgery,the complicated anatomical structure of the lung tissue and more variation lead to the difficulty of the operation and the relatively high risk.The young doctor is difficult to understand it.So we made the patient’s lung model by three-dimensional reconstruction and 3D printing technique before operation and introduced the 3D lung model into the clinical diagnosis and treatment and teaching of the Department of thoracic surgery.To discuss the effect of its application.Methods:1.Experimental group: select 2016.11-2017.6 in our hospital with surgical indications,can be performed by thoracoscopy in 20 cases of pulmonary segmental resection,before the operation of the chest CT enhanced examination of the image for three-dimensional reconstruction,and then the 3D lung model was made.According to the 3D lung lobe model,we observed the location of the tumor,set the operative plan and simulated the operation before operation.During the operation,the actual condition of the pulmonary veins was recorded and compared with the 3D lung lobe model,and the operative time was recorded.Control group: 20 cases of 2014.01-2016.09 who underwent lung segmental resection in our hospital were reviewed.The two groups of operation time were compared and analyzed.2.During the period of 2017.5-2017.10,38 interns in Department of thoracic surgery were randomly assigned to 2 groups,the control group and the observation group,with 19 patients in each group.The control group adopted traditional routine clinical teaching methods,and explained the interns based on the 8 edition of surgical books.In the observation group,the anatomical structure of lung tissue and the process of lung operation were explained by the three-dimensional reconstruction image and the 3D print model combine with the book.And the two groups of interns were all in the form of the degree of the pulmonary anatomy and the operation percentile examination paper record andThe scores of the two groups of doctors were recorded and summarized.Result:1.The coincidence rate between the pulmonary veins in the operation and that in the 3D printing lung lobectomy model was 100%,and no variant blood vessels were found.Experimental group: operation time(115.6 ± 3.9)min.Control group: operation time(135.2.0±3.5)min.Statistical method: using SPSS 22.0 statistical software,using t test analysis,the control group and the experimental group operation time compared to P< 0.05 has statistical significance.2.The two groups of interns took the quiz test.The score of the control group was(77.26±1.40),and the score of the observation group was(89.79±0.73).Statistical method:the SPSS 22.0 statistical software was used.Analysis by group t test.The test scores of the observation group were compared with those of the control group,P < 0.05 was statistically significant.Conclusion:1.Compared with the segmental resection of 3D lobectomy model and conventional non 3D model,the operative time was reduced.The 3D model was almost entirely consistent with the actual pulmonary vein.The segmental veins displayed in the3 D model were all ligated during operation to avoid the risk of ligation of pulmonary segmental veins.During the operation,the time to identify the veins of pulmonary segments was omitted,and the experimental results were achieved.2.The 3D lung lobe model is superior to the traditional teaching mode in the examination results.The interns fully understand the teaching content of the 3D model,achieve the purpose of learning,and reduce the difficulty of learning in the Department of thoracic surgery.At the same time,the 3D lung model can achieve similar results with the teaching of cadaveric specimens,making up for the lack of anatomical specimens.It solved the problem of dissecting anatomical specimens and brought innovation to clinical teaching.In a word 3D reconstruction and printing technology has a good application prospect in complex pulmonary surgery and teaching in Department of thoracic surgery. |
