ObjectiveLean management(lean)is a whole set of scientific management mode originated from Toyota production system of Japan.In the early 21st century,some hospitals in China began to apply lean management tools to hospital process transformation projects.Many theories and practical experiences at home and abroad show that applying lean management correctly to hospital practice has obvious effects from the aspects of service flow optimization at the micro level,the improvement of patient satisfaction to the improvement of management at the macro level,and the development of hospital culture.Therefore,it is very important to scientifically and objectively evaluate the overall operation status of lean project,which is the research direction of this topic:(1)Establish the evaluation index system of the maturity of lean medical projects and provide theoretical guidance for the hospital managers to evaluate the maturity of lean medical projects.(2)The evaluation index system of lean medical project maturity was applied to hospital management to explore whether its effect could comprehensively describe the characteristics of lean project maturity.Methods(1)Delphi Method:the Delphi method is used to construct the index system.Researchers should firstly conduct questionnaires seeking indicators of the evaluation index system to experts who have both theoretical and practical experience in lean medical management.Then,revise questionnaires accordingly and conduct them over again.Finally,carry out statistical analysis on the result and build lean health program evaluation index system based on it.(2)Analytic Hierarchy Process(AHP):experts are invited to grade the importance according to the relative importance registration form in ahp.By using the judgment matrix obtained by pairwise comparison of indicators,all data are analyzed by statistical software and each indicator is given the corresponding weight.(3)Empirical Research Method:taking the lean medical project of N hospital as an example,empirical research was conducted to further test the feasibility and effectiveness of the evaluation index system proposed in this study.(4)Fuzzy Comprehensive Evaluation Method:it is a comprehensive evaluation method based on fuzzy mathematics,which transforms the evaluation of lean project in N hospital from qualitative to quantitative analysis and evaluates lean project systematically and scientifically.Results(1)This study using the Delphi method,two rounds of 18 consulting experts,questionnaire recovery rate 100%,and effective sample authority coefficient is 0.865,built a lean health project maturity evaluation index system.The evaluation index system includes project team building,project background and environment support,project operation,project benefit and goal of the project,project satisfaction and continuous improvement of six first-level indicators and 31 secondary indexes.(2)The index is weighted by AHP and the random consistency ratio CR is 0.012,which is less than 0.10.That is to say,there is no logical error in the judgment of the weight of the first-level index,and the consistency of the judgment matrix of the second-level index also conforms to the standard.(3)The evaluation index system can objectively,scientifically and comprehensively evaluate and improve the lean medical projects,which is proved by applying empirical approach and fuzzy comprehensive evaluation approach.By using the evaluation index system to evaluate the project—accelerating group physical examination time of N Hospital,we get an overall score of 8.34 points.Although the project gets a reasonable score,it has room for improvement.Conclusion(1)This study scientifically constructed the evaluation index system of the maturity of lean medical projects,which can objectively,scientifically and comprehensively evaluate the lean medical projects,find the problems existing in the projects,and guide the projects to further adjust the strategies.(2)The example of the evaluation index system proposed in this study was used to verify the rationality and prove that the constructed index system was reasonable and scientific. |
