| Objective: To analyze dexamethasone intravenous injection and nervous around to the influence on the effect of femoral nerve block.Methods:.A total of 90 patients who underwent arthr oscopic meniscus repair inthe Affiliated Hospital of Yan Bian University from January 2019 to December 2020were selected,ASA is classified as gradeⅠ-Ⅱ.Randomly divided into 3 groups,control group for group A,group B,group C,30 patients in each group.A group(control group): the f emoral nerve around 0.375% of ropivacaine injection paid 20ml.Group B(intravenous medication group): the f emoral nerve around 0.375% ofropivacaine injection paid 20 ml,after general anesthesia intubation intravenouscontaining 5 mg dexamethasone 50 ml of 0.9% saline water.Group C(drug around thenerve group): 0.375% ropivacaine 20 ml containing dexamethasone 5m g was injectedaround the femoral nerve.All patients were treated with ultrasound-guided femoralnerve block combined with general anesthesia.The duration of sensory block in threegroups was observed and recorded(Defined as operation area from the end of theinjection to be the first to feel the pain of time),and the Mean Arterial Pressure(MAP)and Heart Rate(HR)before operation,30 minutes after operation and 60 minutes afteroperation and the duration of the motion block were observed and recorded.Postoperative 4 h、8 h、12 h pain related results and dizziness、nausea、vomiting afteroperation、postoperative infection、nerve palsy after surgery and patients’ satisfactionof anesthesia.Results:1.The three groups of patients with intraoperative hemodynamic comparison,comparison before surgery,surgery after 30 points,the operation began afte r 60 minutes,on average,no difference between the three groups of patients(P> 0.05).2.Compared to the time of three groups of patients with sensory blockade,the results show significant difference(P< 0.05).Specific performance in: compared with the group A,the sensory blocking time of the Group B and C is obviously extended(P<0.05).Group B’sensation blocking time is significantly extended compared to the C group(P< 0.05).3.Compared to the time of three groups of patients with movement block,the results showed that there was no statistically significant difference(P> 0.05).4.The three groups of patients after 4 hours 、8 hours、12 hours of NRS pain score,according to the results : Three groups of patients aft er 4 hours of NRS pain score have no obvious difference(P> 0.05).Group B and C groups of patients with postoperative8 hours of NRS scores were significantly lower than that of group A,the difference was statistically significant(P< 0.05).Group B after 8 hours of NRS score was lower than that in group C,the difference was statistically significant(P< 0.05).Group B and group C after 12 hours of NRS score was lower than that in group A,the difference was statistically significant(P< 0.05).Differences in group B and group C have no statistical significance(P> 0.05).5.Of the three groups of patients ’ with postoperative adverse reaction conditions,no significant difference(P> 0.05).6.On the comparison to three group s of patients satisfaction with anesthesia,the difference between group A and group C has no statistical significance(P> 0.05).The satisfaction of group B was obviously higher than that of group A and group C,the difference was statistically significant(P< 0.05).Conclusion:.Compared with perineural injection of dexamethasone,intravenous injection of dexamethasone can prolong the time of sensory block and analgesia of femoral nerve block,effectively reduce the degree of pain in the early stage after operation,and get the best anesthetic effect. |
