Analysis Of Relevant Clinical Characteristics Of Acute Exacerbation In Chronic Obstructive Pulmonary Disease Patients With Non-small Cell Lung Cancer

Abstract/Summary:
Purpose: 1.To study the possible risk factors of cell lung cancer in patients with chronic obstructive pulmonary disease(COPD).2.To investigate the effect of non-small cell lung cancer on acute exacerbation of chronic obstructive pulmonary disease(AECOPD)in order to reduce acute exacerbation,improve the prognosis and improve the quality of life of such patients.Methods: 1.The general data,occupational exposure history,smoking index,pulmonary function data,complications,m MRC score,CAT score,pathological type of lung cancer,lung cancer site,lung cancer stage,inflammation-related indicators of COPD patients diagnosed with NSCLC and COPD patients alone in Northern Jiangsu Provincial People’s Hospital from January 1,2018 to October 31,2020 were collected and analyzed by SPSS 22.0 software.The normal distribution measurement data were expressed in the form of(?)± s.The analysis of variance was used.The non-normal distribution measurement data were expressed as the median and the non-parametric test was used.The enumeration data were expressed as the rate and analyzed by Pearson chi-square test or fisher exact test.P < 0.05 was considered statistically significant.2.Sixty-two COPD patients diagnosed as NSCLC by histopathology and pulmonary function were selected,and 84 COPD patients diagnosed as simple COPD by pulmonary function were selected as the control group.The statistical differences in the general data(gender,age,weight,height),occupational exposure history,smoking index,pulmonary function data, complications and other data between the two groups were compared,and the data with statistical differences were further analyzed by logistic regression to analyze the independent risk factors of NSCLC in COPD patients.3.The statistical differences between COPD patients with NSCLC and COPD patients alone in the frequency of exacerbations as well as the risk assessment grade of exacerbations were further analyzed.4.COPD patients with NSCLC were divided into high frequency group(AECOPD frequency > 1 time within 1 year)and low frequency group(AECOPD frequency ≦ 1 time within 1 year)according to the frequency of AECOPD within 1 year after the diagnosis of both.The general data(gender,age,weight,height),smoking index,pulmonary function data,inflammatory markers(total white blood cell count,total neutrophil count,total eosinophil count,C-reactive protein),pathological type of lung cancer,lung cancer site,lung cancer stage and other indicators were compared between the two groups.The data of the two groups were compared for statistical differences.Results: 1.The data analysis of COPD with NSCLC and COPD showed that there was no significant difference in age,body mass index,gender,occupational exposure history and m MRC score between the two groups(P > 0.05),and the two groups showed advanced age(68 ± 6.42 VS 74 ± 6.94),body mass index(Kg/m2)within the normal range(22 ± 3.48 VS 22 ± 2.93),more males than females,more occupational exposure history,and a certain degree of dyspnea(1.9 ±0.96 VS 2.1 ± 0.95);while there were significant differences in smoking index,FEV1 percentage of predicted value,FEV1/FVC and CAT score between the two groups,and in COPD group with NSCLC,smoking index,worse pulmonary function and CAT score.2.The analysis revealed that smoking(OR = 7.03,95% CI = 1.11 – 44.3,P =0.038),FEV1/FVC(OR = 0.90,95% CI = 0.83 – 0.98,P = 0.016),and CAT≧ 17 points(OR = 2.87,95% CI = 1.45 – 5.68,P = 0.003)were risk factors for NSCLC in COPD patients.3.The frequency of AECOPD within 1 year in the COPD group with NSCLC was 2 times,of which the proportion of patients with frequency > 1 time in the COPD group with NSCLC was 77.4%,and the proportion of patients with acute risk assessment grade D was the highest(43.5%);while the frequency of AECOPD within 1 year in the COPD alone group was 1 time,of which the proportion of patients with frequency > 1 time in the COPD alone group was40.5%,and the proportion of patients with acute exacerbation risk assessment grade A was the highest(42.9%).There were statistical differences in the above three indicators in the COPD simple COPD group with NSCLC.4.Gender,body mass index,smoking index,FEV1,FEV1% pred,FEV1/FVC in COPD patients with NSCLC high frequency and low frequency were not statistically significant,but the two groups considered normal body mass index,male,smoking,FEV1 percentage of predicted < 50%,FEV1 decreased,FEV1/FVC decreased rare.There were statistical differences in age,inflammatory inflammatory,pathological type of lung cancer,lesion site and lung cancer stage among COPD patients with NSCLC at high frequency and low frequency 68 markers,and the high frequency group showed age ≧ 10 years,white blood cell count ≧ 10 × 10 ^ 9,neutrophil count ≧ 6 × 10 ^ 9,eosinophil count ≧ 0.15 × 10 ^ 9,C-reactive protein ≧ 10 ng/L,squamous cell carcinoma and lung cancer stage III-IV,P < 0.05.Conclusion: 1.Smoking,poor lung function and high CAT score are the risk factors of lung cancer in patients with chronic obstructive pulmonary disease.2.COPD patients with NSCLC have a higher frequency of exacerbations and are more likely to have exacerbations.3.Age,inflammatory parameters,and stage III-IV lung squamous cell carcinoma may be risk factors affecting the frequency of acute exacerbations in COPD patients with NSCLC.
