Purpose:To observe the clinical effectiveness and security of Yuebi decoction plus Banxia in the treatment of acute exacerbation of chronic obstructive pulmonary disease(the type of phlegm-heat congestion the lung). Methods:Sixty inpatients from the ward of the Department of Respiratory and Critical Care Medicine,Linyi Hospital of Traditional Chinese Medicine, who were diagnosed by Modern medicine as having AECOPD and identified by traditional Chinese medicine as having phlegm-heat congestion in the lung, were divided into a control group and a Chinese medicine group by SPSS24 random number generator, with 30 patients in each of the two groups. The control group was provided with conventional Western medicine, and the Chinese medicine group was provided with Yuebi decoction plus Banxia at the foundation of the control group, and both groups were given a treatment duration of 10 days.Total TCM evidence score, single score of major symptoms,WBC,NEUT%,CRP,PCT,ESR,CAT score,mMRC score, blood gas analysis indexes(PaO2,PaCO2),and pulmonary function indexes(FEV1%pred,FEV1/FVC)were used as observation indexes,by comparing the changes of the above indexes in the control group and the Chinese medicine group before and after treatment, and then evaluated the clinical efficacy of the application of Yuebi decoction plus Banxia in the treatment of AECOPD patients who has the syndrome of phlegm-heat congestion in the lung.Blood, urine and stool routine, liver function, kidney function and electrocardiogram were used as safety indicators.Recording the adverse reactions that occurred in patients during the trial to evaluate the safety of this trial.Outcome:1.General data comparison:the gender and age of the two groups of patients were comparable, and the difference between them was not statistically significant(P>0.05).2.Total TCM scores:compared within the two groups, the total TCM scores of patients in both groups decreased after treatment compared with those before treatment, and the difference was statistically significant(P<0.01);compared between the two groups, the total TCM scores of patients treated by the Chinese medicine group decreased to a greater extent than those in the control group, and the difference was statistically significant(P<0.01)3.Chinese medicine evidence efficacy rate:90%of the effective rate of the Chinese medicine group was significantly higher than 70%of the effective rate of the control group, and the difference was statistically notably(P<0.05).4.CAT score:compared within the two groups, the CAT scores of patients in both groups decreased after treatment compared with those before treatment, and the difference was statistically significant(P<0.01);compared between the two groups, the CAT scores of patients in the Chinese medicine group improved better than those in the control group after treatment, and the difference was statistically significant(P<0.01).5.mMRC score:both groups decreased after treatment, and the difference was statistically significant(P<0.01);compared between the two groups, the mMRC score in the Chinese medicine group improved better than that in the control group after treatment, and the difference was statistically significant(P<0.05).6.Levels of inflammatory factors:compared within the two groups, the levels of WBC,NEUT%,CRP,PCT and ESR decreased in both groups after treatment compared with those before treatment, and the differences were statistically significant(P<0.01);compared between the two groups, the levels of inflammatory factors decreased in patients in the Chinese medicine group after treatment were higher than those in the control group, and the differences were statistically significant(P<0.01).7.Blood gas analysis、pulmonary function indexes:compared within the two groups, the oxygen partial pressure、carbon dioxide partial pressure、FEV1%pred,FEV1/FVC of patients in both groups improved after treatment, and the difference was statistically significant(P<0.01);compared between the two groups, there was no significant difference in the improvement of above indicators in both groups after treatment, and the difference was statistically significant(P>0.05).8.Safety:No adverse reactions occurred in the two groups during the trial, and no clinically significant deterioration of safety indexes was observed.Concolusion:The treatment of AECOPD(phlegm-heat obstructing lung syndrome)with Yuebi and Banxia decoction can effectively improve the clinical symptoms of patients, improve the quality of life of patients, and have no adverse reactions and good safety.
