| Objective:The formation of Deep venous thrombosis DVT after surgery in patients with moderate risk of varicose veins was observed by using drug prevention and physical prevention measures.The differences in the incidence of DVT and the impact on coagulation function of patients with varicose veins were compared between the two different preventive measures.The clinical significance and influence of two different preventive measures were analyzed and compared to provide theoretical basis for the prevention of postoperative DVT in such patients in the future clinical work.Methods: This was a prospective,single-blind,randomized controlled study.The subjects were 140 patients with moderate risk of thrombosis who were admitted to the vascular surgery department of the Fujian Medical University from May 17,2019 to December 7,2019.All the patients agreed to participate in the study.Among them,64 were males and 76 were females.Relevant preoperative examinations were completed and basic data of patients were collected,such as height,weight,BMI,blood routine,liver and kidney function,coagulation function,D-dimer,previous medical history(hypertension,diabetes),CEAP classification,Caprini risk score.According to inclusion and exclusion criteria,140 patients were randomly divided into two groups,namely 70 patients in the experimental group and 70 patients in the control group.In the experimental group,there were 29 males and 41 females,with an average age of55.76±6.75 years.The control group included 35 males and 35 females,with an average age of 54.09±7.93 years.The patients characteristics between the two study groups were not statistically different.In the experimental group,patients were treated with dabigatran(110mg/day)for preventive anticoagulation during the 5 days.While patients in the control group were treated with intermittent pneumatic compression for thromboprophylaxis.Coagulation function and D-dimer were examined in all patients on the 1st and 5th day after the operation.on the 30 th day after the operation,we will reexamined patients’ coagulation function,d-dimer and lower limb vein color ultrasound to determine whether there was DVT formation and the changes of coagulation function and D-dimer in the patients after the operation.Results: In the experimental group,there were 3 cases of DVT,the incidence rate was 4.3%,all of which were asymptomatic calf muscular venous thrombosis.In another group,there were 10 cases of deep venous thrombosis of lower extremity,with an incidence of 14.3%,and all of them had clinical symptoms.The incidence of DVT in the control group was higher than that in the experimental group,and the difference was statistically significant(P < 0.05).Conclusions: For patients with varicose veins in the lower extremities at risk of moderate risk thrombosis,dabigatran on postoperative DVT was better than that of intermittent pneumatic compression.And Dabigatran prolonged TT in patients,but had no effect on PT,INR,APTT and FIB. |
