| ObjectiveTo observe the clinical effect and recurrence rate of treating liver-stomach-disharmony gastroesophageal reflux disease(GERD)by using traditional Chinese medicine and western medicine sequential therapy. To compare with the curative effect which was only treated by western medicine. To sum up the advantage and provide evidence for the application of using traditional Chinese medicine and western medicine sequential therapy to treat GERD.MethodAccording to the principle of randomization, control and single blindness, the 72 subjects, who were diagnosed liver-stomach-disharmony GERD form the outpatient and inpatient departments of Baoan Chinese traditional medicine Hospital in Shenzhen,Guangdong Province between May 2019 and October 2020,were collected according to the inclusion and exclusion criteria. After informed consent,72 subjects were divided into the control group and therapy group evenly by using a random number table method. The control group only used Western medicine treatment(esomeprazole magnesium enteric-coated tablets, taken daily throughout the day). The therapy group used traditional Chinese medicine(taken daily throughout the day)and western medicine(esomeprazole magnesium enteric-coated tablets, taken daily after group treatment to 2 weeks of treatment, taken every other day from 3 weeks of treatment to 4 weeks of treatment, taken 2 days apart from 5 weeks of treatment to 8 weeks of treatment). To observe the related clinical indicators of the two groups patients before treatment, after the treatment,4 weeks after withdrawal and 8 weeks after withdrawal. Then we used the Symptom and score of traditional Chinese medicine(TCM)symptoms and Gastro-esophageal-reflux-disease questionnaire(Gerd Q)to get the score. To analyse the clinical data by statistical analysis. Then to compare the total curative effect, symptom improvement and recurrence rate between the two groups,so that a conclusion was drawn.Result1.General information:All 72 patients which were included completed the study process. Before treatment, there were no significant difference in the distribution of gender, age, and disease duration between the two groups of patients(P>0.05).There was no significant difference in the classification of gastroscopy results between the two groups of patients before treatment(P>0.05).The overall balance is good and comparable.2.(1)Clinical efficacy:After 8 weeks of treatment, there was significant difference in clinical efficacy between the two groups of patients(P<0.01).The total effective rate in the control group was 80.56%,and the total effective rate in the treatment group was 100.00%.The total effective rate of the treatment group was better than that of the control group.Compared with the control group, there was significant difference in the total effective rate of the therapy group(P<0.05).(2)Total scores of TCM symptoms:Before treatment, there was no significant difference in the total scores of the two groups of patients(P>0.05).After treatment, the total scores of the two groups of patients were significantly reduced(P<0.01).And the total scores of the therapy group decreased significantly more than that of the control group(P<0.01).There was a significant difference in the total scores between the two groups after treatment(P<0.01).The therapy group improved the overall symptoms of TCM better than the control group.(3)Main symptoms of TCM:Before treatment, there were all no significant difference in the degree grading of each main symptom of the two groups of patients(P>0.05).After treatment, the the degree grading of each main symptom of the two groups of patients were all significantly reduced(P<0.01).And the therapy group was better than the control group in improving acid reflux and heartburn(P<0.01).The therapy group was similar to the control group in improving post-sternal burning pain and belching(P>0.05).(4)Secondary symptoms of TCM:Before treatment, there were all no significant difference in the degree grading of each secondary symptom of the two groups of patients(P>0.05).After treatment, the degree grading of each secondary symptom of the two groups of patients were all significantly reduced(P<0.01).And the therapy group was better than the control group in improving dry mouth, bitterness, epigastric pain, abdominal distension, upset and insomnia(P<0.05).The therapy group was similar to the control group in improving pharyngeal discomfort, hunger, night cough, anorexia, fatigue, loose stools, and constipation(P>0.05).3.Recurrence rate:(1)4 weeks after withdrawal:The recurrence rate in the control group was 10.34%,and the recurrence rate in the therapy group was 5.56%.There was no significant difference in the recurrence rate between the two groups(P>0.05).(2)8 weeks after withdrawal:The recurrence rate in the control group was 41.38%,and the recurrence rate in the therapy group was 13.89%.Compared with the control group, there was significant difference in the recurrence rate of the therapy group(P<0.05).(3)Compared with 4 weeks after withdrawal, the recurrence rate of 8 weeks after withdrawal was significantly increased in the control group(P<0.01),and the recurrence rate of 8 weeks after withdrawal was not significantly increased in the treatment group(P>0.05).ConclusionThe treatment of GRED by using traditional Chinese medicine and western medicine sequential therapy has unique advantages in improving the total effective rate of treatment, improving reflux and related TCM symptoms, and reducing the recurrence rate.Compared with the western medicine group alone, its comprehensive effect was better, which deserved further clinical research and application. |
