| Objectives:Observe the evaluation of WBC,CRP,ESR and other metabolic indexes in patients with type 2 diabetes,to explore the effects of the above inflammatory factors on the occurrence and development of Type 2 diabetes and the prognosis of cardiovascular and cerebrovascular diseases.Methods:A retrospective analysis of 7,925 patients with type 2 diabetes diagnosed in the Ninth Affiliated Hospital of the Guangxi Medical University between January 2015 and December 2019 was performed,the eligible cases were collected according to the criteria of case inclusion and exclusion,then they were divided into two groups: Group B(521 cases,type 2 diabetes mellitus as the main diagnosis,no cardiovascular and cerebrovascular disease)and Group C(502 cases,type 2 diabetes mellitus,had cardiovascular and cerebrovascular disease during the course of disease),in addition,517 healthy patients were selected as control group A.All three groups were divided into three groups to complete the history,age,sex and other general information and blood routine white blood cell(WBC),erythrocyte sedimentation rate(ESR),c-reactive protein(CRP),total cholesterol(TC),triglyceride,high-density Lipoprotein,Low-density Lipoprotein,fasting blood glucose(FBG),blood pressure,body mass index(BMI)and other indicators collection,spearman was used to analyze the correlation between white blood cell(WBC),c-reactive protein(CRP),erythrocyte sedimentation rate(ESR)and other metabolic indexes in type 2diabetes patients with cardiovascular and cerebrovascular accident,ROC curve was constructed to analyze the predictive efficiency of inflammatory factors WBC,ESR and CRP in the occurrence of cardiovascular and cerebrovascular complications in type 2 diabetes.Results:1.There was no significant difference among the three groups in the level of sex,age and course of disease(P > 0.05).2.There were differences in WBC,ESR,CRP,TC,TG,HDL,LDL and FBG between groups,and the P values were all less than 0.05.The differences were statistically significant,and the above indicators showed a gradually increasing trend among groups A,B and C(HDL,on the other hand).The differences in systolic blood pressure,diastolic blood pressure and BMI between groups were not statistically significant,while the P values were greater than 0.05;3.Compared with group A,there were differences in ESR,CRP,TC,TG,HDL,LDL and FBG in group B,all with P values less than 0.05,and the differences were statistically significant.Moreover,ESR,CRP,FBG,TC,TG and LDL in group B were all higher than that in group A,while HDL was lower than that in group A.Compared with group A,there were differences in WBC,ESR,CRP,TC,TG,HDL,LDL and FBG in group C.The P values were all less than 0.05,and the differences were statistically significant.In addition,WBC,ESR,CRP,FBG,TC,TG and LDL in group C were all higher than that in group A,while HDL was lower than that in group A.Compared with group B,there were differences in WBC,ESR,CRP and FBG in group C(P < 0.05),and the differences were statistically significant.Besides,WBC,ESR,CRP and FBG in group C we-re all higher than those in group B;4.Spearman was used to analyze the correlation between WBC,CRP,ESR,blood lipid,blood pressure,FBG and BMI in group C.It was found that WBC was positively correlated with TC,TG,LDL,systolic blood pressure and FBG.The P values were all less than 0.05,and negatively correlated with HDL,There was no significant correlation with diastolic blood pressure and BMI,and the P values were all greater than 0.05.CRP was positively correlated with TC,TG,LDL,systolic blood pressure and FBG,all with P values less than 0.05,and negatively correlated with HDL,P values was less than 0.05,and no significant correlation with diastolic blood pressure and BMI,all with P values greater than 0.05.ESR was positively correlated with TC,TG,LDL,and systolic blood pressure all P values were less than 0.05 and negatively correlated with HDL,P < 0.05,but not significantly correlated with diastolic blood pressure,FBG,and BMI(all P values were greater than 0.05).The three inflammatory factors mentioned above were correlated with different metabolic indicators,but the correlation was weak,WBC and CRP in Group C were positively correlated,P value was less than 0.05.5.ROC curve was used to analyze the predictive efficiency of inflammatory factors WBC,ESR and CRP in the occurrence of cardiovascular and cerebrovascular complications in type 2 diabetes.Among the above-mentioned inflammatory factors,the area under the curve of WBC was0.615,the sensitivity was 61.8%,the specificity was 63.5%,the yodeng index was 0.253,and the optimal pointcut value was 6.575.The area under the ESR curve is 0.725,the sensitivity is 79.3%,the specificity is 64.6%,the youden index is 0.439,and the optimal pointcut value is 24.860.The area under the curve of CRP was 0.708,the sensitivity was 78.3%,the specificity was 61.7%,the youden index was 0.400,and the optimal pointcut value was 6.950.Conclusions:The main inflammatory factors:WBC,CRP and ESR,were correlated with type 2 diabetes Mellitus(T2DM),and had predictive value for cardiovascular and cerebrovascular diseases in T2 DM. |
