Application Value Of Endoscopic Hemostatic Clip Combined With Drug Injection In The Treatment Of Duodenal Ulcer Forrest Ⅱa Bleeding

Objective: To investigate the application value of endoscopic hemostatic clip combined with injection of epinephrine and haemocoagulase agkistrodon in the treatment of duodenal ulcer Forrest Ⅱa bleeding.Methods: A total of 74 cases of duodenal ulcer Forrest Ⅱa bleeding admitted to The Second Nanning People’s Hospital from April 2015 to October2019 were collected.They were divided into two groups.The combined group:endoscopic hemostatic clip combined with injection of epinephrine and haemocoagulase agkistrodon group(n=34);the hemostatic clip group: endoscopic hemostatic clip clamping group(n=40).For patients with different endoscopic treatment methods,immediate hemostasis,rebleeding within 3 days,rebleeding within 5 days,surgery,postoperative complications,ulcer lesion diameter and curative effect,endoscopic treatment costs,length of hospitalization,hospital costs are observed and recorded.Results: The immediate hemostasis rate of the combined group and the hemostatic clip group were 100%,and the difference was not statistically significant(P>0.05).The rebleeding rate within 3 days of the combined group was 2.94%,which was significantly lower than that of the hemostatic clip group(20%),the difference was statistically significant(P<0.05).In terms of rebleeding rate within 5 days,the combined group was 2.94%,which was less than 15% of the hemostatic clip group,the difference was not statistically significant(P>0.05).There was no significant difference in the surgery rate and the incidence of postoperative complications between the combined group and the hemostatic clip group(P>0.05).The endoscopic treatment cost of the combined group is higher than that of the hemostatic clip group,and the length of hospitalization is shorter than that of the hemostatic clip group.The difference is statistically significant(P<0.05).For the hospitalization cost,there is no statistical difference between the combined and hemostatic clip groups significance(P>0.05).Conclusion: Endoscopic hemostatic clip combined with injection of epinephrine and haemocoagulase agkistrodon in the treatment of duodenal ulcer Forrest Ⅱa bleeding,not only has a high immediate hemostasis rate,but also can effectively reduce the patient’s rebleeding rate within 3 days and can also shorten the length of hospital stay,and does not increase the cost of hospitalization of patients,which is worthy of clinical promotion and application.
