Design Of Cardiopulmonary Rehabilitation Diagnosis And Treatment System Based On Mobile Health

Objective:To understand the development status and existing problems of cardiopulmonary rehabilitation and mobile health,and to clarify the requirements of healthcare professionals for cardiopulmonary rehabilitation and mobile health and the related needs of patients.And to provide a scientific and convenient diagnosis and management system of cardiopulmonary rehabilitation.Method:1.The qualitative method was used to determine the research object through the purpose sampling and according to the maximum difference principle.From September 2019 to December 2019,the healthcare professionals of cardiology department,respiratory department and rehabilitation department and patients with cardiopulmonary disease were selected for qualitative interview.Through the face-to-face semi-structured interview to collect data.The theme analysis method was used,and the interview data were sorted and analyzed in Nvivo12.2.Through the expert working group meeting,the contents of rehabilitation assessment,rehabilitation training and patients’ health education were adjusted and revised,the basic framework of the diagnosis and treatment scheme was demonstrated,and preliminarily construct the cardiopulmonary rehabilitation diagnosis and treatment scheme.3.Through two rounds of Delphi expert consultation,all the module settings of the two ports of the diagnosis and treatment system and the contents of the knowledge base were demonstrated.The specific content of each role module and the specific items of the knowledge base were determined,and the revision of the diagnosis and treatment scheme was basically completed.4.Through one-to-one expert consultation,the design of the module,the specific items of the knowledge base and the final review and confirmation of the use path,the preliminary development of the cardiopulmonary rehabilitation diagnosis and treatment system based on mobile medicine was completed.Results:1.Construction of cardiopulmonary rehabilitation diagnosis and treatment system(1)Through sorting and analyzing the interview data of 14 healthcare professionals and 8 patients,it was found that although most of them have a positive attitude towards cardiopulmonary rehabilitation and mobile health.It also defined the needs of healthcare professionals for standardized rehabilitation process,patients for disease and rehabilitation related knowledge,and healthcare professionals and patients for simple and easy-to-use mobile health.However,the application of cardiopulmonary rehabilitation and mobile health is still facing some difficulties due to various factors.Through the analysis and integration of the data,five themes were formed,namely "cognition of cardiopulmonary rehabilitation"(Theme 1),"obstacles of cardiopulmonary rehabilitation"(Theme 2),"assistance of cardiopulmonary rehabilitation"(Theme 3),"dilemma of mobile health"(Theme 4)and "users’ needs"(Theme 5).Among them,theme 1 includes two sub themes of "Awareness and knowledge" and "Perceived effect",while theme 2 includes five sub themes of "Lack of standardized process","Lack of continuous follow-up","Insufficient health professionals ","Pressure of cost" and "Poor compliance".Theme 3 includes five sub themes: "Education and guidance","Professional training"," Guidance of guidelines "," Facilities " and "Communication between patients".Theme 4 includes three sub themes: " Sense of burden"," Sense of use" and " Preference for traditional health ".Theme 5 includes four sub themes: "Multidisciplinary team","Easy to use","Health knowledge" and " Supportive policies ".(2)The preliminary framework of cardiopulmonary rehabilitation diagnosis and treatment system was proposed.The framework included two ports.The healthcare professionals side included two parts: rehabilitation evaluation and rehabilitation prescription.The rehabilitation evaluation included four aspects and the rehabilitation prescription included six aspects.And the patient side included three parts.(3)Through the expert working group meeting,the functional modules of the 5roles of the 2 ports were preliminarily determined.Among them,the interface of rehabilitation doctor consisted of 6 modules,the interface of therapist and nurse consisted of 3 modules,and the interface of patient consisted of 3 modules.In the knowledge base,there were 3 new items in the "knowledge base of rehabilitation assessment" and 5 new items in the "knowledge base of health related information ".2.Revision of cardiopulmonary rehabilitation diagnosis and treatment systemThe demonstration of basic framework module and knowledge base,after the first round of expert consultation,the healthcare professionals added 4 items and revised 5 items.On the patient side,added 1 item.In the knowledge base,"knowledge base of rehabilitation assessment" added 1 item,and "knowledge base of health related information" revised 1 item.After the second round of expert consultation,1new modules and 2 items were added to the side of healthcare professionals,and 4items were retained in the "my" module of the patient’ side.In the knowledge base,"knowledge base of health related information" deleted 2 items.After two rounds of expert consultation,106 items in the module and 110 items in the knowledge base were determined.3.Determination and development of cardiopulmonary rehabilitation diagnosis and treatment systemThrough one-to-one expert consultation,the specific modules,knowledge base and path map of the diagnosis and treatment system were reviewed and confirmed.On the healthcare professionals’ side,the rehabilitation doctor interface includeed 6modules,the interface of therapist and nurse included 3 modules.The main contents are "evaluation prescription" and "rehabilitation prescription",supplemented by "registration","prescription view","completion of diagnosis and treatment" and "patient information management".And the patient’ side included 3 modules,"home page" is the most important part,and "questionnaire evaluation" and "my" are the supplements.The path map of the user using app was determined,the interface design of the software was improved,and the cardiopulmonary rehabilitation diagnosis and treatment system based on mobile health was determined and developed.Conclusion:1.Although cardiopulmonary rehabilitation lacks a unified standard,mobile health also has some design,use problems and other multiple obstacles,but the vast majority of healthcare professionals and patients have a positive attitude towards cardiopulmonary rehabilitation and mobile health.Healthcare professionals have a high demand for professional guidance and standardized rehabilitation process reference of cardiopulmonary rehabilitation,while patients want to get more health information about disease and rehabilitation.Both sides also made clear the importance of developing simple and easy-to-use software.According to the guidance of domestic and foreign guidelines,expert consensus and professional books,we should provide information support for healthcare professionals and patients,optimize the system design,pay attention to the usability and usefulness of software,and provide scientific and convenient cardiopulmonary rehabilitation diagnosis and treatment system.2.Through expert consultation,we revised and sorted out the specific items of 9modules and 4 knowledge bases in the two ports,providing a systematic and information-based rehabilitation treatment platform for healthcare professionals,and providing patients with various forms of health information including text,pictures and videos.3.The cardiopulmonary rehabilitation diagnosis and treatment system based on mobile health was finally determined and developed,including 6 modules of "registration","evaluation prescription","rehabilitation prescription","prescription view","complete diagnosis and treatment" and "patient information management" on the medical side,and 3 modules of "home page","questionnaire evaluation" and "my" on the patient side.At present,the software is still in the debugging stage,and its usability and usefulness still need to be further tested.
