Objective: To explore the effects of blood flow restriction combined with aerobic training on the glucose and lipid metabolism indexes and vascular endothelial factors of type 2 diabetes,and compare it with traditional low-intensity and high-intensity aerobic training.It aims to evaluate the effectiveness of this exercise method in the treatment of diabetes,and to find a safe and effective exercise method for diabetes patients to treat diabetes and prevent diabetes complications.Methods:A total of 46 patients with type 2 diabetes who were 50-65 years old at the Maigaoqiao Community Health Service Center in Qixia District,Nanjing,with a course of disease of 2-10 years were taken as the experimental subjects.A total of46 people were stratified and randomly sampled into three groups: Low-intensity aerobic group(LI group,40%HRR,n=16),low-intensity aerobic training combined with blood flow restriction group(LI-BFR group,40%HRR+50%AOP,n=13),highintensity aerobic exercise group(HI group,70% HRR,n=17),the exercise cycle was12 weeks,3 times/week,six groups each time,each group had 5 minutes of aerobic treadmill training,with 1 minute rest between groups.Among them,in the LI-BFR group,the subject's thighs were bound with a compression belt in advance,and the compression belt was inflated 5 seconds before the start of the treadmill exercise to ensure that the treadmill exercise and blood flow restriction were performed at the same time.At rest,pressurize and deflate with compression belt,repeat six sets.Throughout the exercise,the subject is at the target exercise intensity by adjusting the damping on the power car,and the Polar heart rate meter is used to detect the heart rate.Blood pressure and blood sugar are measured 15 minutes before and after each exercise to ensure the safety of the subject.Carry out glucose metabolism indicators(Fasting plasma glucose(FPG),Glycosylated hemoglobin Al C(Hb Alc),Fasting insulin(FINS),Homeostasis model assessment insulin resistance(HOMAIR)),lipid metabolism indicators(Total cholesterol(TC),Total cholesterol(TG),High density lipoprotein-cholesterol(HDL-C),Low density lipoprotein-cholesterol(LDL-C),Free fatty acid(FFA)),vascular endothelial factor indicators(Vascular endothelial growth factor(VEGF),Endothelial nitric oxide synthase(e NOS),Endothelin-1(ET-1),nitric oxide(NO))before and after 12 weeks of training.Results:(1)Compared with before training,the FPG,Hb Alc,TG,FFA,e NOS,VEGF,and LI-BFR groups in the three groups changed significantly in TC,NO,HOMA-IR and HOMA-IR and FINS in the HI group(P<0.01);The levels of TC and NO in LI group and HDL-C in LI-BFR group and TC and HDL-C in HI group were significantly changed compared with those before the experiment(P<0.01).Among the three groups,LDL-C,ET-1 and,FINS,HDL-C,LDL-C in LI-BFR groups,and HOMA-IR of LI group were not significantly different(P > 0.05).(2)The comparison between groups found that there were significant differences in FPG and NO between the HI group and the LI group(P<0.05);the LI-BFR group and the LI group showed a very significant difference in FPG(P<0.01).Compared with the LI group,the TC,FFA,e NOS,NO and VEGF concentrations of the LI-BFR group were significantly different(P<0.05).Compared with the HI group,the LIBFR group had significant differences in glucose and lipid metabolism indexes and vascular endothelial factor indexes.There was no significant difference(P>0.05).Conclusions:(1)Compared with the Low-intensity aerobic group,the Highintensity aerobic group had significant differences on improving blood sugar,insulin,cholesterol,triglycerides,free fatty acids,and vascular endothelial relaxing factor.(2)The combination of low-intensity aerobic treadmill training and blood flow restriction can lower blood sugar,lower lipids,and regulate vascular endothelial relaxing factors,and even achieve the effect of high-intensity aerobic training.
