Abstract/Summary:
Patient care can be fragmented due to the complexity of hospital systems. Many potentially preventable hospital readmissions are frequently a result of ineffective communication. The clinical nurse leader role is essential in integrating patient-centered care interventions to promote high quality care and enhance communication. Discharge follow up phone calls is an intervention that clinical nurse leaders connect with patients after discharge to assess the patient's compliance and understanding of their discharge instructions and intervenes if needed. The recommendation within this paper is to incorporate discharge follow up phone calls as a patient-centered care intervention to enhance communication, improve outcomes, and decrease preventable readmissions in the hospital setting.
