The Wartburg Adult Care Community encompasses a Skilled Nursing Facility (SNF), an Adult Day Health Services (ADHS) and an Enriched Housing Program (EHP). Each program is regulated by a different set of rules and regulations administered by the New York State Department of Health and/or the New York State Department of Social Services. Different types of medical record management systems, lack of formal training, absence of policies and procedures, deficiencies in the Accounting of Disclosure Log process and different workflow environments all contribute to obstacles faced by each program when releasing patient health information.;A Release of Patient Health Information Training Program was researched and created in an effort to standardize policies and procedures throughout the campus. Additional objectives of the training program are to: (1)Present the laws the employees should be familiar with when releasing patient health information; (2)Teach the employees the difference between a designated record set and a legal health record; (3)Educate the employees on how to recognize a valid authorization; (4)Familiarize the employees with the access and amendment rules; (5)Teach the employees about the importance of an Accounting of Disclosure Log.
