Study On The Pilot Construction Of Long-Term Care Insurance System In Chengde

With the gradual deepening of the aging of the population,the number of disabled elderly began to expand,and the demand for long-term care increased;since the implementation of the family planning policy in the 1980 s,China's family structure has undergone major changes,with the increase of small-scale families and the increase of women's employment rate,which have led to the weakening of the traditional family care function,and the responsibility of long-term care is from home The court transferred to the society.As a risk arrangement for the disabled elderly to pay for long-term care expenses and receive long-term care services,long-term care insurance has relatively perfect theoretical and practical experience in foreign countries.Foreign practical experience shows that the longterm care insurance has significant effect on meeting the long-term care needs of disabled elderly,alleviating the financial risk of disabled families,and reducing the deficit of government medical care security expenditure.In June 2016,the Ministry of human resources and social security of the State Council issued the guiding opinions on the pilot of long-term care insurance system,which carried out the pilot work of long-term care insurance system in15 cities such as Chengde.As one of the first pilot cities of long-term care insurance system in China,Chengde has made a detailed system arrangement in system system,guarantee level,service content,service organization,fund raising and other aspects.In this paper,the implementation of the long-term care insurance system pilot in Chengde city was investigated on the spot,and its implementation,effect and problems were studied in detail.Compared with other domestic pilot cities,Chengde has a special economic and social geographical environment,such as its low level of economic development,relatively imperfect infrastructure,weak awareness of public insurance,and its proximity to Beijing,resulting in a large number of young and middle-aged labor outflow.All these facts have led to the fact that Chengde has relatively narrow coverage,fund linked with raising income,payment treatment and expenditure linkage,encouraging home care and actively practicing Internet plus.To some extent,the implementation of the long-term care insurance system in Chengde is limited.However,under the cooperation of Chengde Municipal government and Pacific Insurance,the pilot of the long-term care insurance system in Chengde has achieved good results.The pilot effect of the system has significantly reduced the economic burden andbusiness burden of the disabled and their families,and promoted the local medical,pension and nursing industry Development.There are still many problems behind the good implementation effect,such as the system is not perfect,the coverage is not comprehensive,the quality of nursing service is low,the audit cost is high,and it is difficult.In view of the existing problems in the process of implementation and the possible problems in the process of gradual promotion,we have studied the compulsory long-term care insurance system in Germany,the social long-term care insurance system in Japan and the commercial long-term care insurance system in the United States to find solutions,and proposed the joint construction and improvement of the long-term care insurance system by multiple subjects Department,expand the scope of system guarantee,set up special working groups,improve the income level of nursing service personnel,strengthen the training of nursing service skills,set up the scoring mechanism of nursing service,and encourage the participation of the third-party evaluation institutions.Due to the short time and insufficient data of the system pilot,this paper only makes qualitative analysis on the implementation,effect and problems of the Chengde pilot.There is a lack of research on the situation of other domestic pilot and comparative analysis between the pilot,quantitative analysis of policy effect,quantitative calculation of financing mechanism and payment mechanism,etc.in order to continue in the future Continue to carry out in-depth and specific research.
