Objective:Hyperuricemia?HUA?is a symptom of high serum uric acid concentration due to abnormal uric acid metabolism. In recent years, the incidence of HUA has increased due to the improvement of the economic level and the changes in lifestyle, and it is becoming more and more common among young people.Gout is the most common symptom of HUA patients. At the same time, abnormal serum uric acid is related to many diseases such as hypertension, diabetes, obesity, and metabolic syndrome. In recent years, the effects of exercise therapy on various diseases have been researched extensively.Aerobic exercise combined with resistance training is the recommended method for the prevention and treatment of various chronic non-communicable diseases, and the effects have been confirmed gradually. However, the research on the exercise of HUA is still not abundant, and there are still contradictions about the effect of exercise methods and programs on the treatment of HUA.It was been reported that exercise can reduce serum uric acid or increase uric acid level to aggravate the disease. For this reason, the purpose of this study is to research the effects of high-speed resistance training combined with aerobic exercise intervention on serum uric acid in HUA patients, and explore its mechanism from the perspective of oxidative stress. We hope to provide practical solutions for HUA's exercise health management and provide experimental evidence for clinical exercise guidance for HUA patients. Methods:The subjects were divided into a control group and an experimental group randomly, with 11 persons in each group.Subjects received exercise intervention and health education in the experimental group.Subjects only received health education and maintained daily activities in the control group. The form of exercise intervention contains high-speed resistance training combined with aerobic exercise. We controlled the exercise intensity at a medium level through heart rate and subjective fatigue. The subjects should exercise 3 times every week for 12 weeks. Before and after the experiment, the indicators of the two groups were measured and evaluated.Mental and psychological indicators include the WHOQOL-BREF questionnaire and the SWLS scale;the body shape indicators include height, weight, waist circumference, hip circumference,BMI,and WHR;physiological function indicators include blood pressure, vital capacity,VO2max and relative VO2max;blood biochemical indicators include SUA,SCr,BUN,SOD,MDA,eNOS,eGFR,and lipids.Result:1.Safety:During the 12-week exercise intervention, there were no problems such as exercise-induced gout attacks, muscle injury or cardiovascular diseases, indicating that the exercise intervention is safe and suitable for HUA patients.2.Changes in serum uric acid:After exercise intervention, the serum uric acid concentration in the experimental group decreased extremely?p<0.01?,and the difference was very significant compared with the control group?p<0.01?.There was no significant change in uric acid concentration in the control group before and after intervention.Correlation analysis showed that the uric acid value was positively correlated with waist circumference and TG before intervention?p<0.05?,and positively correlated with WHR?p<0.01?.The change rate of uric acid before and after the intervention was negatively correlated with the change rates of VO2max,SOD and eGFR?p<0.05?and positively correlated with the change rates of SCr,BUN,TG and TC?p<0.01?.3.Changes in physiological function indicators:After exercise intervention,VO2max,relative VO2max and vital capacity of the experimental group increased significantly?p<0.05?,and there was no significant change in blood pressure. There was no significant change in the control group.4.Changes in oxidative stress and eNOS:After exercise intervention, the activities of SOD and eNOS in the experimental group increased significantly?p<0.05?,and there was no significant change in MDA concentration. There was no significant change in the control group.5.Changes in renal function indicators:After exercise intervention, the concentrations of SCr and BUN in the experimental group decreased extremely?p<0.01?,which was significantly different from the control group?p<0.05?.eGFR increased significantly?p<0.05?.There was no significant change in the control group.6.Changes in blood lipids:After exercise intervention, the TG concentration in the experimental group decreased significantly?p<0.05?,the TC and LDL-C concentrations decreased extremely?p<0.01?,which were significantly different from the control group?p<0.05?.HDL-C did not change significantly. There was no significant change in the control group.7.Changes in human morphological indicators:After exercise intervention, the indicators of the experimental group did not change significantly. In the control group, the weight and BMI were increased significantly?p<0.05?.Waist circumference, hip circumference, and WHR were not changed significantly.8.Changes in psychological indicators:After exercise intervention, the scores of the experimental group in the physical field of WHOQOL-BREF improved extremely?p<0.01?.The psychological field improved significantly?p<0.05?,which was significantly different from the control group?p<0.05?.The environmental field improved extremely?p<0.01?,which was significantly different from the control group?p<0.05?.There was no significant change in the social relationship field and SWLS scale. There was no significant change in the control group. Conclusion:The 12-week high-speed resistance training combined with aerobic exercise intervention can reduce the serum uric acid concentration of HUA patients by improving the body's antioxidant capacity and vascular endothelial cell function, regulating blood lipid levels, and enhancing renal excretion of uric acid. This prescription is effective and suitable for patients.
