Clinical Observation Of Zhenjing Dingzhi Mixture In The Treatment Of Post-stroke Depression

Objective:Through the clinical observation of patients after stroke,study of post-stroke depression(post-stroke depression,PSD)incidence and related factors,to explore the susceptible factors of PSD and study of the TCM syndromes investigation of patients after stroke at the same time.And further study of the Zhenjing Dingzhi mixture curative effect of treatment of post-stroke depression,and help early diagnosis and intervention treatment of post-stroke depression later.Methods:Subsidiary in the period from April 2018 to December 2018,outpatient and hospital patients of post-stroke of Longhua Hospital affiliated to Shanghai university of traditional Chinese medicine encephalopathy and Fengxian branch of Longhua Hospital,in line with the inclusion and exclusion criteria as the object of study,the general clinical data(including age,sex,BMI),duration of stroke,neurological deficit scores(NIHSS),activities of daily living scale score(ADL),modified Rankin scale assessment,Mini-mental state examination(MMSE)and stroke disease TCM climate survey,and then use patient health questionnaire depression scale(PHQ-9)rating stroke patients divided into PSD and non-PSD group,and statistical data analysis of the relevant factors of the incidence of PSD syndrome and the discrepancy of different TCM climates.Then randomize patients with post-stroke depression,respectively medication of Zhenjing Dingzhi mixture and fluoxetine,And before medicine,taking medicine for four weeks,taking medicine for eight weeks,we assess the effect between two groups of patients using Hamilton Depression Rating Scale(HAMD),Mini-mental State Examination(MMSE),Pittsburgh Sleep Quality Index(PSQI)and Fatigue Severity Scale(FSS)for observing the HAMD deducted rate observed,in order to observe the effect for the treatment of post-stroke depression of Zhenjing Dingzhi mixture.All data analysis using SPSS21.0 for processing.Results:1)A total income of in patients with stroke 120 cases of,of of which the the patients with PSD 52 cases,the incidence rate of PSD is 43.3%.2)Gender,diabetes mellitus,NIHSS score,ADL score,modified Rankin scale,OCSP parting,blood stasis and qi deficiency of TCM climates are relevant factors of PSD.3)PSD occurrence is related to the higher NIHSS score,ADL score,MMSE score.4)The total effective rate of Zhenjing Dingzhi mixture is 78.3%.The total effective rate of fluoxetine is 66.7%.Zhenjing Dingzhi mixture have certain curative effect in PSD,and of the curative effect between Zhenjing Dingzhi mixture and fluoxetine nearly in sleeping and fatigue.Conclusion:The incidence rate of PSD is higher,related to gender,diabetes mellitus,NIHSS score,ADL score,modified Rankin scale,OCSP parting,uncorrelated obviously to age and duration of stroke.Patients who accord with blood stasis and qi deficiency of TCM climates are likely to occur PSD.Zhenjing Dingzhi mixture have certain curative effect in PSD.
