| OBJECTIVE:This study through the observation of Chinese medicine Xiaohuangtongluo powder topical treatment of acute gouty arthritis clinical curative effect of the acute phase,the preliminary discussion on the drug to improve disease joints redness,swelling,heat,pain and assist to reduce the effect of related indicators of inflammation,and to evaluate its safety and provide the basis for further application of the drug,to promote Chinese medicine external treatment in the acute phase of gout treatment of reasonable application.METHODS:Study design to follow the blind state of blind state evaluation,parallel-group,randomization,screening in September 2019 to January 2020 rheumatism of Xiyuan hospital outpatient and ward in patients with acute gouty arthritis,according to the inclusion criteria included 62 cases of acute gouty ar-thritis patients,using SAS9.2 statistical software to generate random Numbers table,by a third party test subjects can be divided into the treatment group and the control group(n=32)(actually completed:31 cases in treatment group,control group 30 cases).The course of treatment is 7 days,in the treatment group,the medicine was applied once a day for 6 hours(8am-2pm),although the control group received external Voltaren(diclofenac diethylamine emulsion)3 times a day(8am,10am,2pm).Changes in VAS score,skin temperature,T-CM syndrome score,serum c-reactive protein(CRP),erythrocyte sedimentation rate(ESR)and total effective rate of patients in two groups were observed before and after treatment,and their safety was analyzed to monitor possible adverse reactions.SPSS26.0 statistical software was used to analyze and process all the data.RESULTS:The first one is in general efficacy.The treatment group was 93.55%better than the total effective rate of the control group was 80%,and the difference between the two groups has statistically significant.The second is the external medication of the two groups could effectively reduce the skin temperature of the affected area,and the treatment group was better than the control group in reducing the skin temperature by comparing the slope of the line graph drawn by the average temperature before treatment,the first day,the third day and the seventh day after treatment.The third one is the pain scores of the two groups decreased significantly after treatment compared with before treatment,and the difference between the two groups has statistically significant.The relief of joint pain in the treatment group was better than that in the control group.The fourth one is the TCM syndrome scores of the two groups were significantly lower than before treatment,and the difference in joint color,joint swelling and total score between the two groups has statistically significant.The effect of improving joint color and joint swelling in the treatment group was better than that in the control group.The fifth one is both ESR and CRP in the two groups were significantly lower than before treatment,indicating that both groups could effectively reduce the inflammatory response with external medication.The last one is blood routine,urine routine,liver and kidney function were compared before and after treatment in the two groups,with no obvious abnormal changes and no adverse reactions.CONCLUSIONS:Through clinical observation,Xiaohuangtongluo powder can effectively alleviate the main symptoms such as joint pain,redness and dysfunction,and can effectively inhibit the inflammatory reaction of the diseased joint,reduce the skin temperature of the affected area,and have no obvious adverse reactions.Therefore,the application of Xiaohuangtongluo powder in the treatment of acute gouty arthritis has a good clinical effect and is worthy of further application. |
