Preliminary Study On The Distribution Of TCM Syndrome Elements In Diabetes Complicated With Depression

Objective:Based on the syndrome differentiation system of syndrome elements,we plan to study the distribution of the common TCM syndrome factor in the state of diabetes complicated with depression through the clinical research,as well as the location and nature of this disease and the distribution of syndrome elements were explored,and the syndrome types were explored through the syndrome elements.Methods:This study adopts the method of cross-sectional study of clinical investigation.First,judge the depression of patients with diabetes.Second,collect basic information and the clinical syndromes of traditional Chinese medicine of the included patients,the method based on syndrome factor differentiation of syndrome factor differentiation,and analyze TCM syndrome factor distribution through statistical method.Third,explore status of diabetic depression syndrome.Results:75 patients of diabetes mellitus with depression were included in this study.1.The depression of 75 diabetic patients with a total of 774 syndrome factor,the "?" syndrome take the most part(40.05%),"?" and "?" syndrome factor accounted for 62.79%,visible diabetic patients with depression symptoms may be more apparent.2.Distribution of single syndrome elements:?disease location syndrome elements:in the frequency of occurrence,the liver appears the most,followed by kidney,mind and spleen,followed by stomach,heart and lung,and the bladder appears the least.According to the average weight comparison of syndrome elements,the syndrome elements whose mean weights were greater than 20 included liver,kidney and spleen,among which the mean weights of liver was the highest;the mean weights of mind were close to 20.disease nature syndrome elements:on the frequency,the highest Yin deficiency,qi stagnation,followed by wet,heat,Yang deficiency,qi deficiency,again for Yang and kang,blood deficiency,phlegm,blood stasis;the frequency of blood heat,wind jin deficit is less;According to the average weight comparison of syndrome elements,Yin deficiency>qi stagnation>qi deficiency>Yang deficiency>wet>blood deficiency>hot>blood stasis>sputum>Yang hyperactivity,except for Yang hyperactivity,the average weight of other syndromes was greater than 20.3.Combination distribution of syndrome elements:?combination of disease location syndrome elements:multiple combinations of disease location and syndrome elements are relatively common,among which 4 combinations of disease location are the most common,accounting for 32.00%."liver,spleen,kidney and mind","liver,heart,kidney and mind",are the main ones.In the second place,there were 3 combinations of disease sites(18.67%),mainly the combination of "liver,kidney and mind","liver,spleen and kidney".The combination of two disease sites(17.33%)was mainly seen in "Iiver and spleen","Iiver and mind" and "liver and kidney".Liver is involved in most combinations,and often appears in different combinations with mind,kidney and spleen syndrome elements·?combination of disease nature syndromes elements:a total of 73 cases(97.33%)of the combination of virtual and real syndromes participated in the combination,and the combination was more complex and varied,among which the combination of 6 and 7 disease nature syndromes elements accounted for the highest proportion(16%),followed by the combination of 8 disease nature syndromes elements(14.67%).Yin deficiency and qi stagnation often participated in the combination of syndromes and elements,with a total of 63 cases,accounting for 84%.The deficiency syndrome element was mainly Yin deficiency,and often appeared in different combinations with other deficiency syndrome elements(qi deficiency,Yang deficiency,blood deficiency).There were 58 cases,accounting for 77.3%.There were 62 cases,accounting for 82.67%,in which qi stagnation was the main syndromes,and other syndromes(Yang and kang,dampness,phlegm,blood stasis,and heat)appeared in different combinations.4.Distribution characteristics of syndrome elements:(Din gender,the occurrence rate of syndrome elements of Yang deficiency in women is higher.?In age,the occurrence rate of spleen and Yang deficiency in the elderly is higher than that of the non-elderly.?With the increase of the course of diabetes,the frequency of stomach and blood deficiency syndrome gradually increased.with the increase of depression score,the heart,mind,stomach,qi deficiency,Yang kang syndrome element more likely to appear,in which the mind,Yang kang syndrome element.5.The combination of syndromes of diabetes mellitus with depression is complex and changeable,which makes it difficult to form objective and standard syndromes.The cluster analysis is used to cluster TCM syndromes.Category one:liver,Yin deficiency,qi stagnation,wet;liver yu Yin deficiency and wet syndrome-The second category:heat,Yang deficiency,kidney,mind,qi deficiency,spleen;spleen and kidney deficiency,heat disturbance mind.Category three:stomach,heart,blood stasis,blood deficiency,phlegm,yangkang;deficiency of heart blood,phlegm and blood stasis.The fourth category:lung,blood heat,wind,fluid deficit,bladder;lung heat and fluid deficit.Conclusion:1.The disease location is mainly in the liver,closely related to the spleen,kidney and mind;disease is the basic deficiency standard,qi and blood Yin and Yang deficiency,qi stagnation,wet,hot,blood stasis,phlegm solid evil.The basic pathogenesis of diabetes mellitus complicated with depression is qi stagnation,Yin deficiency.2.The pathological manifestations of diabetes with depressive state syndrome elements are more serious.and the combination is complex and changeable.Women with diabetes and depression are more likely to have the syndrome of Yang deficiency.The elderly are prone to spleen Yang deficiency.The longer the course of diabetes,the more prone to stomach and blood deficiency syndrome.The more obvious the state of depression,the more prone to the presence of mind,Yang kang syndrome element.3.According to the results of cluster analysis and clinical experience,it can be roughly divided into the following syndrome types:syndrome of liver stagnation,Yin deficiency and damp;syndrome of spleen-kidney Yang deficiency;syndrome of heat disturbing mind;deficiency of heart blood;syndrome of mutual phlegm and blood stasis;syndrome of deficiency of lung heat and fluid.
