Objective:The purpose of this study was to investigate the correlation between serum uric acid level and carotid atherosclerosis in the elderly(? 60 years old),as well as TCM syndrome types and gender, age, smoking,The correlation between alcohol consumption,BMI,and comorbidities provides objective evidence for TCM syndrome differentiation of elderly patients with elevated blood uric acid(UA? 420 ?mol/L),and clarifies the relationship between blood uric acid levels and carotid atherosclerosis.Relationship, improve the clinical efficacy of clinical diagnosis and treatment of elderly patients with elevated carotid atherosclerosis, achieve the purpose of timely and effective control of blood uric acid levels, delay the progression of carotid atherosclerosis, and reduce the incidence of cardiovascular and cerebrovascular diseases in the elderly.Method:A total of 326 elderly patients with carotid atherosclerosis were collected from the outpatient department and inpatients of the Oriental Hospital of Beijing University of Chinese Medicine.100 elderly patients with blood uric acid level ? 420 ?mol/L were included in the observation group, and the remaining 226 patients had blood uric acid levels<420 ?mol/Elderly patients of L were included in the control group, and according to the "Guiding Principles for Clinical Research of New Chinese Medicines(2002)" edited by Zheng Xiaoyu,"Diagnostics of Traditional Chinese Medicine(Second Edition)" edited by Zhu Wenfeng, and"National Standards of the People's Republic of China" issued by the State Technical Supervision Bureau·Traditional Chinese Medicine Diagnosis and Treatment Terminology Syndrome(GB/T16751-1997)and the instructor's years of clinical experience, based on the main symptoms and secondary symptoms, developed five basic syndrome types:phlegm turbid obstruction syndrome,qi stagnation blood stasis syndrome,qi deficiency blood stasis syndrome,Liver and kidney yin deficiency syndrome, spleen and kidney yang deficiency syndrome.Develop a unified questionnaire, collect basic patient information, four diagnosis information, past history, carotid ultrasound results, blood uric acid levels, etc.,and organize data. All data were statistically analyzed using SPSS 22.0 statistical software.Results:1.Hypertension was the most common disease in the observation group, followed by diabetes, coronary heart disease and old cerebral infarction.Among them,81 patients with hypertension were observed in the combination, and 156 patients with hypertension were observed in the control group. The proportion of patients with hypertension was significantly higher than that in the control group. The difference was statistically different, indicating that the elderly with elevated blood uric acid Patients with carotid atherosclerosis have a higher incidence of hypertension than the normal group, and elevated blood uric acid can increase the incidence of hypertension.2.There were 30 patients with smoking history and 70 non-smokers in observation group. There were 56 patients with a clear smoking history in the control group and 170 patients without smoking. After inspection, there was no significant difference in smoking history between the observation group and the control group, indicating that smoking or not had no significant effect on blood uric acid levels.3.In the observation group, the patients with blood uric acid levels of 420?480 ?mol/L were the most, accounting for 65%,and the patients with blood uric acid levels>600 lunol/L were the least, accounting for 3%.The results showed that in elderly patients with elevated serum uric acid, the blood uric acid was mainly mild to moderately elevated4.The distribution trend of the maximum carotid plaque length in the observation group and the control group:observation group>control group. After inspection, the results are statistically different. It shows that with the increase of blood uric acid level, the maximum carotid artery plaque length also increases.5.Among the 326 elderly patients with carotid arteriosclerosis, blood uric acid level was significantly positively correlated with age and BMI.6.Among 326 elderly patients with carotid sclerosis, the serum uric acid level was significantly positively correlated with the maximum plaque length of the carotid artery and the maximum plaque thickness of the carotid artery.7.The distribution trend of TCM syndrome types in elderly patients with elevated carotid atherosclerosis is phlegm turbidity suppression syndrome>liver and kidney yin deficiency syndrome>Qi stagnation and blood stasis syndrome>Qi deficiency and blood stasis syndrome>spleen and kidney yang deficiency syndrome.8.In the observation group, with different syndrome types and related diseases, syndromes with hypertension are more common with phlegm suppression, syndromes with coronary heart disease are more common with qi deficiency and blood stasis syndrome, and syndromes with diabetes are liver Syndromes of kidney yin deficiency are more common.Syndromes with obsolete cerebral infarction and stagnation of phlegm and stasis of blood are more common. After testing, the syndrome types have significant differences between patients with diabetes(P<0.01).9.In the observation group, there were 24 patients with unilateral carotid plaque lesions,58 patients with bilateral carotid plaque lesions, and 18 patients with carotid intima-media thickening. In elderly patients with carotid arteriosclerosis with elevated serum uric acid, patients with sputum turbid suppression were excluded in each syndrome type, and bilateral carotid plaque lesions were significantly more than unilateral lesions.Arterial plaque lesions are more common. The results were statistically different, indicating that the syndrome types had significant differences in the occurrence of carotid unilateral and bilateral plaques(P<0.01).10.In the observation group of phlegm turbidity suppression syndrome,qi stagnation and blood stasis syndrome,qi deficiency and blood stasis syndrome, liver and kidney yin deficiency syndrome, spleen and kidney yang deficiency syndrome and other five syndrome types,The carotid intima-media thickness, the maximum carotid plaque length, and the maximum carotid plaque thickness were significantly positively correlated.Conclusion:1.Elevated blood uric acid can increase the incidence of hypertension in patients.2.Smoking history had no significant effect on blood uric acid levels.3.The blood uric acid level in elderly patients with elevated blood uric acid is mainly moderate to moderate, ranging from 420 to 540 ?mol/L.4.The blood uric acid level was significantly positively correlated with the carotid maximum plaque length and maximum plaque thickness.5.Elderly patients with elevated blood uric acid have the most common syndrome of phlegm turbidity, followed by the syndrome of deficiency of liver, kidney and yin.6.There are significant differences in the distribution of carotid artery plaques between the traditional Chinese medicine syndromes of elderly carotid arteriosclerosis.7.Serum uric acid level in patients with phlegm-turbidity suppression type was significantly positively correlated with carotid artery intima-media thickness, carotid artery maximum plaque length, and carotid artery maximum plaque thickness.
