OBJECTIVEAnalyze and explore the related risk factors and modern Chinese medicine etiology through case-control research methods,provide theoretical basis for the prevention and treatment of thyroid nodules,and provide practical basis for medical staff to provide health guidance and clinical care for patients.METHODSThe patients from the outpatient department of the Department of Endocrinology,Dongzhimen Hospital,Beijing University of Chinese Medicine,and the patients from the Chinese Medicine Outpatient Department of Beijing Medical University were selected from March 1,2019 to December 31,2019.According to the enrollment,exclusion and elimination criteria,200 patients with thyroid nodules were selected as the case group,matched according to age and gender,and 200 patients without thyroid nodules were selected as the control group.Collect the information content of the research subjects in the form of a questionnaire,including basic information:age,gender,height,weight,BMI(Body Mass Index,BMI),marital status,education level;psychological factors include:emotional(peaceful,irritable,irritable,depressed,Anxiety)and stress;eating habits:salt use,dietary taste5 dietary preferences,seafood consumption,eating out;sleeping habits:time to fall asleep,length of sleep;lifestyle:smoking,drinking;past history includes:other thyroid diseases(Hashimoto's thyroiditis,hyperthyroidism,hypothyroidism,thyroid cancer),other diseases(breast hyperplasia,uterine fibroids,ovarian cysts,cysts or nodules in other parts)and thyroid Family history of diseases,a total of 21 items in 7 categories.And collect the thyroid ultrasound examination results and thyroid function test data of the case group.The obtained data,using Excel tables to establish a database,and analysis through SAS9.4 statistical software.RESULTS1 Basic characteristicsIn this study,the frequency matching method was used to match age and gender,so the distribution of age and gender factors between the two groups was the same.A total of 200 cases were included in the case group,with a minimum age of 14 years,a maximum age of 81 years,and an average age of 44.9±12.91 years;a control group of 200 cases,a minimum age of 20 years,a maximum age of 69 years,and an average age of 44.1±12.04 years.The majority of the respondents were concentrated in the three age groups of 31-40 years old(26%),41-50 years old(24%),51-60 years old(25.5%),mainly young and middle-aged,and Women are the majority,accounting for 85%.Chi-square test showed no significant difference in marital status(P=0.2107),education level(P=0.7175),BMI(P=0.6556)(P>0.05),the demographic characteristics of the two groups were well balanced,So it is comparable.2 Univariate analysisMobile phone charging position,time to fall asleep,length of sleep,salt use,dietary taste,emotions,stress,other thyroid diseases,other diseases,family history of thyroid disease are all risk factors for thyroid nodules.Emotional disorder,the prevalence of thyroid nodules increased(?2=73.2892,P<0.0001);the greater the pressure,the higher the prevalence of thyroid nodules(?2=56.7623,P<0.0001);alternate consumption of iodized salt increases the prevalence of thyroid nodules(?2=7.5851,P<0.0225);diet tastes salty,spicy,and sweet,The prevalence of thyroid nodules increased(?2=16.3199,P<0.0001);the later the time to fall asleep,the higher the incidence of thyroid nodules(?2=18.9189,P<0.0001);the more the sleep time Short,increased prevalence of thyroid nodules(?2=14.9229,P<0.0001);the closer the mobile phone charging position is to the human body,the higher the incidence of thyroid nodules(?2=4.9368,P=0.0263);combined with other thyroid diseases,the prevalence of thyroid nodules increased(x2=53.4413,P<0.0001);combined with other diseases,the prevalence of thyroid nodules increased(?2=26.1993,P<0.0001);with a family history of thyroid disease,the prevalence of thyroid nodules increased(?2=52.1313,P<0.0001).Smoking,drinking,dietary preferences,eating seafood,and eating outside have no effect on the occurrence of thyroid duration less than 7 hours(OR=2.282,P=0.0164),emotional disorder(OR=4.045,P<0.0001),excessive stress(OR=2.478,P=0.0006),other thyroid diseases(OR=3.794,P<0.0001),other diseases(OR=1.719,P=0.0407),family history(OR=4.355,P<0.0001)),these 7 factors become independent risk factors for the occurrence of thyroid nodular disease.CONCLUSIONThis study found that emotional disorders,high stress,falling asleep after 23:00,sleeping for less than 7 hours,combined with thyroid disease,combined with other nodular diseases,and family history are positively correlated with the occurrence of thyroid nodules.Among them,emotional disorders People who are prone to irritability,irritability,anxiety,and depression are more likely to get thyroid nodules;in other thyroid diseases,people with Hashimoto's thyroiditis and hypothyroidism are more likely to get thyroid nodules;in other nodular diseases,People with breast hyperplasia are more likely to get thyroid nodules.In terms of prevention,we must raise people's awareness of thyroid nodules,focus on regulating emotions,relieving stress,and having regular schedules.The modern cause of thyroid nodules has changed from the ancient water and soil environment to the modern emotional dissatisfaction;secondly,it also includes irregular life and rest,often staying up late,and lack of sleep;modern people's living environment is compared with ancient times.Great changes have taken place,the pace of life has accelerated,and life pressure has increased.Therefore,on the whole,the connotation of the cause of TCM has changed,and we need to examine the time and seek the cause for treatment.
