Meta Analysis Of Acupuncture Treatment Of Aphasia After Stroke And Study On The Rule Of Acupoint Selection

Objective:To objectively evaluate the efficacy of acupuncture in the treatment of post-stroke aphasia through Meta-analysis,and on this basis,to preliminarily obtain acupuncture points selection rules,and provide a basis for clinical application and research.Methods:Search CNKI,VIP,Wangfang database,CBM,PubMed,Cochrane Library,EMBase and other databases related to randomized controlled studies on acupuncture treatment of post-stroke aphasia.The search time limit is up to May 2020,and the two researchers independently screened Literature,extract data,evaluate the included literature with reference to the bias risk assessment tool of the Cochrane Collaboration,and use RevMan5.3 software for Meta analysis.Establish a Microsoft Excel data software table,and use frequency analysis to analyze the use of acupoints included in the literatureResults:After literature screening,a total of 28 literatures were finally determined to be included in the analysis.There were 2121 post-stroke patients with aphasia.26 studies evaluated the effective rate of treatment.Meta-analysis showed that acupuncture treatment was compared with the control group.,Can significantly improve the total effective rate of treatment(OR=4.32,95%CI[3.26,5.73]),the difference is statistically significant(P<0.05).Eight studies recorded the total scores of language function scores.The results of Meta analysis suggest that acupuncture treatment can improve the total scores of language function scores(SMD=1.76,95%CI[0.98,2.54])compared with the control group.There are differences Statistically significant(P<0.05).Fifteen studies recorded the oral expression scores in the language function score sub-item.The results of the meta analysis showed that acupuncture treatment can significantly improve the oral expression level compared with the control group(SMD=1.93,95%CI[1.28,2.59]),The difference was statistically significant(P<0.05).16 studies recorded the listening comprehension scores in the language function score sub-item.The results of the meta-analysis indicated that acupuncture treatment can significantly increase the level of listening comprehension compared with the control group(SMD=1.19,95%CI[0.66,1.721),The difference was statistically significant(P<0.05).Thirteen studies recorded the retelling scores in the subitem of language function scores.The results of the meta-analysis indicated that acupuncture treatment can significantly increase the retelling level compared with the control group(SMD=1.12,95%CI[0.78,1.46]).The difference was statistically significant(P<0.05).Ten studies recorded the naming scores in the language function scoring sub-item.The results of the meta analysis indicated that acupuncture treatment can significantly improve the naming level compared with the control group(SMD=1.11,95%CI[0.70,1.51]]).The difference was statistically significant(P<0.05).Twelve studies recorded the reading scores in the language function score sub-item.The results of the meta analysis showed that acupuncture treatment can significantly improve reading skills compared with the control group(SMD=1.91,95%CI[1.13,2.68]),and there is a difference There is statistical significance(P<0.05).Nine studies recorded the writing score in the language function score sub-item,and the results of the meta analysis showed that compared with the control group,acupuncture treatment can significantly improve the writing level(SMD=1.18,95%CI[0.74,1.62]),there is a difference There is statistical significance(P<0.05).Twelve studies evaluated the changes in the severity of aphasia before and after treatment,and the results of the meta-analysis indicated that the use of acupuncture to treat post-stroke aphasia improved the severity of aphasia better than the control group(SMD=0.98,95%CI[0.62,1.34]),the difference was statistically significant(P<0.05).Ten studies evaluated the improvement of functional communication ability,and the results of Meta analysis indicated that the use of acupuncture to treat post-stroke aphasia improved functional communication ability better than the control group(SMD=2.28,95%CI[1.40,3.16]),The difference was statistically significant(P<0.05).Two studies evaluated the improvement of the quality of life of patients with aphasia before and after treatment,and the results of Meta analysis indicated that the use of acupuncture for post-stroke aphasia improved the quality of life of patients with aphasia better than the control group(MD=15.72,95%CI[11.56,19.89]),the difference was statistically significant(P<0.05).Acupuncture treatment of post-stroke aphasia main acupoint frequency analysis results:the main acupoints involved 57 different acupoints,of which the meridian points were 34,the meridian acupuncture points were 22,and the Ashi point was 1,and the total frequency of use was 145 times.The main acupoints commonly used for acupuncture treatment of post-stroke aphasia are Baihui,Yamen,Lianquan,Language Zone 1,Yuye,Jinjin,Fengfu,Fengchi,Shanglianquan,Fanglianquan,and parietal anterior slash,The three areas of speech,Hegu.The main acupoints commonly used for acupuncture treatment of post-stroke aphasia are the Du Channel,Ren Channel,Foot Shaoyin Gallbladder Meridian,Hand Yangming Large Intestine Meridian,Foot Jueyin Pericardium Meridian,Hand Shaoyang Sanjiao Meridian.Conclusion:Acupuncture has a good clinical effect in the treatment of post-stroke aphasia,can improve language function scores(including spontaneous talk,listening comprehension,retelling,naming,reading,writing and other sub-items scores),and improve the severity of aphasia and aphasia The improvement of patients' functional communication skills and the quality of life of patients with stroke aphasia,but the quality of the literature included in the study is generally poor,so the results of this study are somewhat biased.In future research,it is necessary to collect more high-quality,multi-center,large-sample clinical randomized controlled trials for demonstration.Acupuncture treatment of aphasia after stroke is recommended to take Baihui,Yamen,Lianquan,Speech One,Yuye,Jinjin,Fengfu,Fengchi,Shanglianquan,Penanglianquan,Parietotemporal Anterior Slash,and Speech Three District,Hegu and other points.
