Evaluation Of The Clinical Efficacy Of Electroacupuncture "Xing Shen Tong Tiao" In The Treatment Of Post-stroke Anxiety Disorder

Objective:Based on the theory of Zang manifestation,our study focused on the close relationship between "heart","brain" and "spirit".In order to evaluate the clinical effect of electroacupuncture on mental and physical symptoms,we observed and recorded the changes of HAMA scale,BI scale and SAS scale in patients with post-stroke anxiety disorder,who were treated in the way of "simultaneous treatment of body and spirit" with electroacupuncture.We also discussed the correlation between physical and mental symptoms,which could provide a basis for the traditional Chinese medicine "simultaneous treatment of body and spirit",which was beneficial to help completing acupuncture treatment plan of PSAD and promoting its clinical application.Methods:The study was divided into an experimental group and a control group,with 20 cases in each group.1.Experimental group:(1)Acupoints:Baihui(DU 20),Fengfu(Du 16),Xinyu(BL 15),Shenmen(HT 7).(2)Operation:patients in prone position.DU 20:flatly pierced for 0.5-1 inch;DU 16:pierced slowly into the jaw direction for 0.5-1 inch;BL 15:obliquely pierced for 0.5-0.8 inch to the spine;HT 7:straight pierced for 0.3-0.5 inch.After having the sensation of"Deqi",we performed reinforcing-reducing method on all acupoints.DU 20(negative electrode)and DU 16(positive electrode)were a group of electroacupuncture needles.On the affected body side,BL 15(positive electrode)and HT 7(negative electrode)were a group of electroacupuncture needles.We choosed the dense wave and selected the frequency in 20 Hz,which was absolutely based on the patient's tolerance.Needles were retained for 20 minutes.The treatment was given every other days,3 times a week for 1 month.The efficacy evaluation was performed after the treatment.2.Control group:Patients took 10 mg paroxetine hydrochloride tablets after breakfast in the initial 4 days and started taking 20 mg on the 5th day after breakfast once a day.The treatment lasted for 1 month and the efficacy evaluation was performed after the treatment.3.Observation indicators:Hamilton anxiety scale(HAMA),Self-care Ability Assessment Barthel Index(BI),self-rating anxiety scale(SAS),and safety indicators of patients before and after treatment.Results:1.Before the treatment,according to the analysis,the statistics about patients' gender,age,years of education,HAMA scale scores,physical factors,mental factors,BI scores,SAS scores showed no difference between the experimental group and the control group(P>0.05),which suggested that the two groups were comparable.2.In the overall efficacy,the effective rate of the experimental group and the control group were both 85%,but the markedly effective rate of the experimental group was 65%,compared with 25%in the control group,It suggested that the markedly effective rate of electroacupuncture is better than paroxetine hydrochloride tablets.3.HAMA scale and various factors comparisonComparison within the group:The HAMA scale scores,physical factors,and mental factors in the experimental group and the control group were statistically different(P<0.05),indicating that both therapies had therapeutic effects.Comparison between groups:After the treatment,the HAMA score of the experimental group was statistically different from that of the control group(P<0.05),indicating that the improvement brought by electroacupuncture was better than drug therapy;The electroacupuncture had better therapeutic effects on PSAD patients' mental symptoms and physical symptoms.4.Correlation analysis of scores of physical factors and mental factorsWe made a correlation analysis of changes in physical factors and mental factors before and after treatment in the experimental group(P=0.0036),with statistical significance and the correlation coefficient r=0.61894,which suggested that the physical symptoms and mental symptoms had a positive correlation.With the improvement of mental symptoms,the physical symptoms also had a good improvement.In the control group,the correlation analysis of physical factors and mental factors that we made showed that P>0.05,which was not statistically significant,suggesting that although paroxetine hydrochloride tablets could improve the physical and mental symptoms of PSAD.These factors were not quite closely related,indicating that there might be other factors that were not known to improve the disease and affected the results of the trial in the control group.5.BI scale comparisonComparison within the group:The BI scores of the patients in the experimental group and the control group were statistically different(P<0.05),indicating that both therapies could significantly improve the quality of life of the patients.Comparison between groups:After the treatment,the BI scale score of the experimental group was statistically different from that of the control group(P<0.05).The electroacupuncture helped PSAD patients to restore physical function and improved their self-care ability.6.SAS scale comparisonComparison within the group:The SAS scores of the experimental group and the control group before and after treatment were statistically different(P<0.05),indicating that both treatments could alleviate the subjective anxiety of the patients.Comparison between groups:After the treatment,the SAS scale score of the experimental group was statistically different from that of the control group(P<0.05).The electroacupuncture could effectively improve the subjective anxiety symptoms of PSAD.Conclusions:"simultaneous treatment of body and spirit" with electroacupuncture had a significant clinical effect in treating PSAD.It could not only improve the patient's mental symptoms,but also adjusted the physical symptoms of stroke patients and promoted the recovery of physical function.Through the comparison between groups,the experimental group had better efficacy than the control group and the markedly effective rate was better.Electroacupuncture had good effects on both the physical and mental symptoms at the same time.The treating method of "simultaneous treatment of body and spirit" was worthy of further clinical application.
