Radiation dermatitis is a skin and mucous reaction caused by radiation exposure,which can be seen in patients receiving radiation therapy(hereinafter referred to as:ra diotherapy),sufferers in nuclear leakage events,technicians engaged in radiation-relate d work,and so on.It is inevitable that damages will be caused to the surrounding sk in or mucous while using radiation to inhibit the proliferation of cancer cells.There a re pathological changes include color change,structure damage,barrier function disord er,abnormal nerve reaction,etc.In serious cases,some ulcers are difficult to heal,which cause severe trauma to the body and mind of patients.In recent years,doctors and nurses in the field of tumor pay more attention to radiation dermatitis,and there are abundant related researches.The study of traditional Chinese medicine(TCM)treat ment of this disease is numerous and has the characteristics of effective and cheap.T his article will further explore the role of traditional Chinese medicine in the treatmen t of radiation dermatitis through the introduction of radiation dermatitis research and t he analysis of clinical research results.This paper consists of literature review and clinical research.Part 1 Literature ReviewBased on the literature review of the laboratory and clinical researches of radiatio n dermatitis in the field of traditional Chinese medicine and Western medicine in rece nt years,the present situation of the occurrence of radiodermatitis,the research progre ss of its internal mechanism and the exploration of traditional Chinese and Western m edicine treatment were summarized.Part 2 Clinical ResearchObjective:To observe the clinical efficacy and safety of Yufu formula(main drug s:honeysuckle,Coptis,purslane,Salvia miltiorrhiza,lithospermum,etc.)in the prev ention and treatment of acute radiation dermatitis.To study its influence on the occurr ence time and degree of acute radiation dermatitis and the relief effect on itching,pai n and other symptoms,so as to provide new ideas and methods for clinical treatment.Methods:In this study,40 patients who fit the inclusion criteria were divided int o observation group and control group with 20 cases in each group.Since the second week of radiotherapy,the observation group was treat with Yufu formula,and the co ntrol group with Jingwanhong ointment.Radiotherapy method:linear accelerator was u sed in both groups,6MV X-ray or 8MeV electron wire was used,2.00?2.64gy/d,5 t imes a week,the total dose was 50Gy?66Gy.The patients in both groups were obser ved from the beginning of enrollment to the end of 1 week after radiotherapy.The ra diation dermatitis grade,RISRAS score,pain score,pruritus score and erythema grade were assessed weekly,and the above data were statistically analyzed.Results:The morbidity of acute radiation dermatitis was 75%(P>0.05)in both groups.There was no patient who appeared acute radiation dermatitis of grade ? or ?V in the two groups.The number of cases with grade ? and ? reactions was 11(55%),4(20%)in the observation group,8(40%)and 7(35%)in the control group,respectively.There was no statistical difference between the two groups(P>0.05),b ut the degree of the observation group was lighter than that of the control group.The average occurrence date of acute radiation dermatitis in the observation group was lat er than that in the control group,and the average cumulative dose was higher than th at in the control group,the difference was statistically significant(P<0.05).The RIS RAS score of the observation group was 2.25 points/week lower than that of the cont rol group(3.23 points/week),the difference was statistically significant(P<0.05).Th e scores of pruritus and pain in the observation group were lower than those in the c ontrol group,but there was no significant difference between the two groups(P>0.05).Erythema:in the observation group,there were 11 cases(55%)of grade ? erythem a,4 cases(20%)of grade ? erythema,in the comtrol group,there were 9 cases(45%)of grade ? erythema and 6 cases(30%)of grade ? erythema.Conclusion:Yufu formula can effectively delay the occurrence of acute radiation dermatitis,reduce the degree of it,relieve the pain and itch,and has better safety.
