Analysis Of The Susceptibility Factors Of Traditional Chinese Medicine In The ICU Respiratory System Nosocomial Infection Of Guang'anmen Hospital And The Correlation Study

ObjectivesAnalyze and summarize the susceptibility factors of TCM syndromes in the ICU respiratory system infection of Guanganmen Hospital and the changes of TCM syndrome elements before and after infection,to explore the pathological changes of TCM in hospital infection of respiratory system and provide theoretical support for TCM to prevent and cure respiratory nosocomial infection.MethodsA retrospective study method was used to collect the clinical data of all hospitalized patients from January 2017 to December 2019 of Guang'anmen Hospital infection,which was divided into infection group and control group according to whether or not respiratory nosocomial infection occurred the general situation of each group.TCM syndrome differentiation information,western medicine related information and other information were collected to establish a database,using spss25.0 statistical software for data analysis,induction and summary.Results1.The basic situation:the average infection rate of ICU respiratory nosocomial infection in Guang'anmen Hospital from January 2017 to December 2019 was 14.1%,and the difference in sex,age and nutritional status between the infection group and the control group was not statistically significant.In terms of basic diseases,both groups had many chronic diseases,but the distribution of pulmonary infection,cerebrovascular disease,tumor,chronic heart failure and operation in the case group was higher than that in the control group;in terms of days of hospital,the average days in the infection group was twice as long as that in the control group,and about 10 days of hospitalization was prone to respiratory nosocomial infection;in terms of prognosis,The mortality rate of the infection group is 62.3%,the improvement rate is 30%,the control group mortality rate is 36%,the disease improvement rate is 48%,the infection group has more risk of death than the control group.2.Terms of western medicine susceptibility factors:tracheal intubation,tracheotomy,gastrointestinal catheterization and cerebrovascular disease are independent risk factors for the occurrence of respiratory nosocomial infection.Timely removal of implanted trachea,gastrointestinal tract tube,early ventilator offline training,is expected to delay the occurrence of respiratory nosocomial infection.3.The aspect of TCM susceptibility factors:ICU respiratory infection syndrome is a mixture of deficiency and reality,and the analysis of syndromes finally concludes that Yang deficiency,dampness,kidney and heat are independent risk factors of traditional Chinese medicine,and Yang deficiency is the most dangerous.It indicates that the characteristic of hospital infection of ICU respiratory system include pathology and position.The pathology is Yang deficiency,dampness,and heat.Yang deficiency is the most prominent,and the position is kidney.When the pathological changes of dampness and heat appear in patients with kidney yang deficiency,it is necessary to arouse the attention of medical staff and take effective measures in time.4.The change of TCM syndrome elements:the main elements of TCM syndromes before respiratory nosocomial infection were dampness syndrome,heat and phlegm syndrome,and the distribution frequency of empirical elements was 2.2 times that of deficiency syndrome elements.It shows that the depletion of Qi Yin runs through the development process of respiratory nosocomial infection,and paying attention to Qi Yin protection is helpful to improve the prognosis of patients with respiratory nosocomial infection.5.In terms of pathogenic bacteria:a total of 76 strains of pathogenic bacteria were detected,the specimens were derived from sputum 74 cases,from pleural effusion 2 cases,mainly gram-negative bacteria,the top three pathogens were Acinetobacter baumannii,Pseudomonas aeruginosa,Klebsiella pneumoniae,Acinetobacter baumannii drug has more serious resistance problem in clinical treatment.Conclusions1.ICU respiratory infection has poor prognosis.Tracheal intubation,tracheotomy,gastrointestinal catheterization,and cerebrovascular disease are the Western medical risk factors for ICU respiratory infection.2.The susceptibility factors of TCM for nosocomial infection of respiratory system in ICU show that the pathogenic syndrome is yang deficiency,dampness,and heat.Yang deficiency is the most prominent,and the disease syndrome is kidney.When patients with kidney-yang deficiency have wet or hot pathological changes,care should be taken to prevent the occurrence of nosocomial infections of the respiratory system.3.After hospital infection of respiratory system,the patient's Qi Yin loss is more obvious.Paying attention to the drug resistance and toxicity of bacteria,clearing heat and dispersing heat,and strengthening the solid foundation may be the entry point for traditional Chinese medicine to prevent and treat respiratory infections in hospital.
