The goal and purpose of the research:This study uses a retrospective study combined with a Meta-analysis.Through the data mining instructor's medication experience and rules for treating depressive disorders,the main prescriptions and drug compatibility for depressive disorders were obtained.A meta-analysis of the efficacy and safety of the main prescriptions.Obtain clinical experience and evidence-based evidence to provide evidence for the differential treatment of depressive disorders.Research methods:Data mining:Medical records of depressive patients diagnosed and treated by Chief Physician Han Zhenyun from January 6,2015 to December 4,2019 at the Second Department of Encephalopathy Specialist,Dongfang Hospital,Beijing University of Chinese Medicine.Summarize information such as name,gender,date of birth,date of visit,diagnosis,prescription of Chinese medicine,etc.Standardize and unify the information and establish a data mining database.Use SPSS and Modeler software for cluster analysis and association rule analysis.Meta-analysis:Search databases such as CNKI.VIP.WanFang,SinoMed,PubMed,Embase,and The Cochrane Library.The retrieval time is set to build the database until February 2020.A randomized controlled trial(RCT)of Xiaoyao San(addition and subtraction)in the treatment of mild to moderate depressive disorders with liver depression and spleen deficiency was collected.The data of the included literature were extracted by two researchers,and the methodological quality of the included studies was evaluated by the Cochrane Collaborative Risk Assessment Tool.Meta-analysis was performed using RevMan 5.3 softwareResearch result:Data mining:(1)A total of 336 patients with depression were included in the study,with a total of].425 consultations.110 were male(32.74%)and 226 were female(67.26%).The main ages were 40-59,60-79,and 20-39.(2)A total of 1425 prescriptions and 215 flavors of Chinese medicines were used,and the cumulative frequency of using Chinese medicines was 25912 times.An average of 18.18 flavors of Chinese medicines were used per prescription.According to the application frequency,except for licorice,Chaihu ranked first(1042 times 73.12%).Traditional Chinese medicines with application frequency>20%are licorice,Bupleurum,Angelica,Muxiang,Poria,Mudanpi,Salvia,Gardenia,raw keel,raw oyster,atractylodes,Chuanxiong,Chenpi,raw rehmannia,pinellia,Atractylodes,sand Kernel,Codonopsis sibiricum,Astragalus membranaceus,Beisha ginseng,Chinese jujube kernel,Paeonia lactiflora,acacia flower,melon scallion,pearl powder,floating wheat,yam,sage god,perilla stem,scutellaria baicalensis,and Huang Jing have 31 flavors.Set it as high frequency Chinese medicine.(3)The four qi properties are mainly warm,flat,slightly cold,and cold.The five flavors are mainly sweet,bitter and bitter.The main meridian is the spleen meridian,heart meridian,liver meridian,and lung meridian.From the perspective of efficacy classification,it is mainly tonic medicine,qi-eliminating medicine,heat-clearing medicine,and tranquilizer.(4)The cluster analysis is divided into 7 categories:? liver regulating drugs(suppressing liver,clearing liver,and relieving liver),? spleen strengthening drugs,?sedative drugs(yangxin anshen,Zhuan anshen,ningxin anshen),? Hexue(blood-enriching,nourishing,activating blood)medicine,?stomach medicine,?qi-eliminating phlegm medicine,yin-qi nourishing yin medicine.(5)Analysis of association rules,a total of 1498 rules were obtained.Compatibility of commonly used drugs includes:Pinellia ternata-raw peel,raw keel-raw oysters,Bupleurum-Huang Huang,Bupleurum-Bai Zhi,Muxiang-Amomum.Muxiang-Acacia flowers,Angelica-Chuanxiong,Bupleurum-Muxiang-Angelica.Meta-analysis:A total of 53 RCTs were included in a total of 4269 patients.The results showed that:(1)Compared with SSRls,Xiaoyao San(addition and subtraction)is equivalent in efficacy(OR=1.34.95%CI[0.89,2.03],P=0.16)and HAMD score(MD=-0.51,95%CI[-1.73,0.72],P=0.42);in terms of the number of adverse reactions(OR=0.19,95%CI[0.09,0.41],P<0.0001),Xiaoyao San is superior to SSRIs.(2)Xiaoyaosan(addition and subtraction)combined with SSRIs is superior to SSRIs alone in terms of effectiveness(OR=3.74,95%CI[3.04.4.61].P<0.00001),HAMD score(MD=-2.95,95%CI[-3.96,-1.94],P<0.00001).SDS score(MD=-4.55,95%CI[-5.36,-3.75],P<0.00001),and number of adverse reactions(OR=0.45,95%CI[0.34,0.59],P<0.00001).Research conclusion:Data mining:(1)Depression patients who are seen by the instructor's clinic are more common in women,mainly in the elderly.(2)The instructor's use of traditional Chinese medicine for the treatment of depressive disorders mainly belongs to the spleen,heart,liver,and lung meridians,suggesting that the disease location involves the spleen,heart,liver,lung,and other internal organs;The four coldnesses and coldness are mainly medicine,reflecting the disease of deficiency and solidness,and the mixture of cold and heat;the five flavors are mainly sweet,bitter,and spicy.The efficacy of traditional Chinese medicine is mainly tonic,qi,heat,and tranquilizer.Wetness,phlegm,blood circulation,liver smoothing,astringent medicine,targeting viscera,qi and blood loss of yin and yang(especially lung and spleen qi and yin deficiency,blood deficiency first),qi depression,fire depression,distress,dampness,sputum condensation,Pathogenesis such as blood stasis and hyperactivity.(3)7 types of results of cluster analysis,including regulating liver(suppressing liver,clearing liver,relieving liver),strengthening the spleen,soothing the nerves(reinforcing and relieving the nerves,removing annoying and reassuring nerves,restlessly reassuring the nerves),and blood(tonicate,nourish.Blood circulation),and stomach,qi and phlegm.nourishing qi and nourishing yin.(4)Based on the results of data mining,instructors often choose Bupleurum,Angelica,Poria,Atractylodes,Baishao,Licorice and other drugs to treat depressive disorders,and take"soothing liver,nourishing blood and spleen" as the prescription.Combined with clinical lectures,the instructor treats depression with Xiaoyaosan as the main prescription,liver and spleen co-harmony,supplement and diarrhea,and syndrome differentiation plus or minus medication.Commonly used Pinellia-Orange peel,Raw keel-Raw oyster,Bupleurum-Scutellaria baicalensis,Bupleurum-White peony,Muxiang-sharen.Muxiang-Hehuanhua,Angelica-chuanxiong and other medicine pairs.Meta-analysis:Compared with SSRIs,Xiaoyaosan(addition and subtraction)treatment of mild to moderate depression of liver depression and spleen deficiency is better than SSRIs alone.There are no serious adverse reactions and good safety.Xiaoyaosan is used for clinical treatment Liver depression and spleen deficiency type mild to moderate depression provides evidence-based evidence.However,it is limited to factors such as the low quality of the included studies and the short follow-up time.It is recommended that future related studies should clearly report the version of HAMD used,describe the specific adverse drug reactions and the number of cases,and intervene as much as possible.The time should be 8 weeks or more,and the follow-up time is longer,which makes the clinical research more rigorous..
