| Objects:Diabetic macrovascular complications are one of the important causes of death in diabetic patients,but these patients have no obvious clinical symptoms in the early stage.The carotid artery is a good place for atherosclerosis,and the carotid intima-media thickness is easy to check as the formation of plaque,so carotid atherosclerosis is an important indicator for judging the prognosis of diabetic macroangiopathy.Modern pharmacological studies have shown that Panax notoginseng can interfere with the mechanism of multiple atherosclerosis development.Therefore,by observing the clinical efficacy and safety of patients with type 2 diabetes mellitus complicated with carotid atherosclerosis and blood stasis syndrome after treatment with Panax notoginseng powder,the mechanism is discussed,which provides a new idea for Tradition Chinese Medicine(TCM)syndrome differentiation and treatment of type 2 diabetes with carotid atherosclerosis.A combination of Chinese and Western medicine for the treatment of type 2 diabetes with carotid atherosclerosis provides a reference.Method:Sixty type 2 diabetes mellitus patients with carotid atherosclerosis and blood stasis syndrome were randomized according to certain inclusion criteria and exclusion criteria and were randomized into 30 patients in the treatment group and the control group.The control group was given basic treatments such as regulate blood sugar,blood lipids,and anti-platelet agglutination.The treatment group was given Panax notoginseng powder on the basis of the basic treatment,and 3 months was a course of treatment.A total of two courses were observed.Questionnaires were used to score TCM syndromes and evaluate the symptom improvement of the two groups.Colorectal Doppler ultrasonography was used to examine the carotid intima-media thickness,plaque number and area before and after treatment;Laboratory indicators such as blood,lipids and safety indicators such as blood routine,liver and kidney function.The clinical treatment effects of the two groups of patients were analyzed by statistical methods such as t test and one-way analysis of variance.Results:(1)There were no significant differences in the general conditions,laboratory parameters,carotid atherosclerotic lesions and TCM syndrome scores between the two groups before treatment,which were comparable.(2)After treatment,the TCM symptom scores of the two groups were significantly improved.The treatment group was better than the control group,the effective rate was 86.7%(26/30),with statistical difference(P<0.05).The blood glucose status of the patients improved,but there was no significant difference between the two groups(P>0.05).The blood lipids of the two groups improved after treatment,but there was significant difference between the two groups(P<0.05).Carotid intima-media thickness(CIMT)and plaque area were improved in the latter two groups.The treatment group was better than the control group,and the effective rate was 83.3%(26/30),with statistical difference(P<0.05).(3)There were no abnormalities in the safety indicators during the whole treatment,and no adverse reactions occurred at the same time,indicating that the clinical safety of Panax notoginseng powder is relatively reliable.Conclusion:The instructor was inspired by Professor Lv Renhe's "micro-zhengjia" theory.Combined with the development mechanism of diabetes and many years of clinical experience,the main pathogenesis of patients with type 2 diabetes complicated with carotid atherosclerosis is blood stasis,which is considered to be the main type of syndrome is blood stasis syndrome,so the use of Panax notoginseng powder for the treatment of such patients.The results of the research indicate that Panax notoginseng powder has excellent efficacy in improving symptoms in type 2 diabetes mellitus with carotid atherosclerosis,which can improve the quality of life of patients.It can also control less CIMT,stabilize plaque,and delay atherosclerosis.The course of cirrhosis progresses.It has good safety and has certain clinical application prospects.It is worthy of further systematic research and full use of its therapeutic effects. |
