Objective:Observing the clinical effect and safety of Luomaitong composed of traditional Chinese medicine(TCM) on diabetic patients with carotid atherosclerosis(CAS) to explore its action mechanism, providing scientific evidence for clinical treatment, and providing new methods for ancillary clinical treatment and make up for Western medicine treatment.Methods:50patients that met qualifying criteria for study were randomly assigned to the treatment group and the control group by1:1. The25patients in the control group were treated with foundation treatment, and the25patients in the treatment group were treated with Luomaitong on the basis of foundation treatment, for a course of3months, observing two treatment courses. Evaluating the improvements of the clinical symptoms two sets of traditional Chinese medicine in patients with symptomatic improvement and observing the changes of carotid intima-media thickness(CIMT), carotid plaque, the blood flow, laboratory indicators such as blood glucose, blood lipid, hemoglobin Alc(HbAlc), homeostasis model assessment of insulin resistance(HOMA-IR), C-reactive protein(CRP), urine micro protein/urinary creatinine (m-Alb/u-Cr) and security indicators. All data for statistical analysis using statistical software SPSS15.0.Results:(1) There was no significant difference between the treatment group and the control group before treatment in general information, laboratory indicators, the degree of carotid atherosclerosis and blood flow, and traditional Chinese medicine (TCM) symptom. The two groups are comparable.(2) Statistical analysis of the data shows that TCM symptom integral is positively correlated with the degree of carotid atherosclerosis and blood flow.(3) Curative effect analysis results:0The clinic syndromes of treatment group and control group were all improved(P<0.05), but the curative effect of treatment group was better than control group(P<0.05).(2)The CIMT and the plaque area of two groups were all reduced compared with the station before treatment and the incidence of plaque in treatment group was lower than the control group, but there was no statistical difference between two groups (P>0.05). Analysed with efficiency, the total effective rate of treatment groupwas84%, and the control group was56%. There was a statistically significant difference of two sets (PO.05).〤ommon carotid artery(CCA) resistance index(RI) was all reduced both groups, and the diastolic peak velocity was all improved both groups(P<0.05). The treatment group was significantly better than the control group (PO.05).■The blood glucose, blood lipid, HbAlc, HOMA-IFL and CRP of both groups were all improved compared with the station before treatment(P<0.05). There was statistical difference of HDL-C^Lp(a)^HOMA-IR^CRPbetween two groups after treatment(P<0.05).(4)Security analysis results:During the course of treatment in both groups, there were less adverse reactions, so the medicine is safe.Conclusion:Tutor proposed that "liver and kidney deficiency and phlegm-stasis blocking collaterals" is the common pathogenesis of diabetes mellitus and carotid atherosclerosis, insulin resistance is their common soil. Traditional Chinese medicine Luomaitong has the effect of treating different diseases. Our research also shows that Luomaitong could effectively relieve clinical symptoms to improve the quality of patients’life, control the thickening of CIMT, delay the progress of carotid atherosclerosis and assist vulnerable plaque. Besides, it could effectively reduce CCARI, improve the diastolic peak velocity, reduce blood glucose and blood lipid levels, and reduce insulin resistance and inflammation. There were less adverse reactions. It can be applied, and worth to further study.
