Low back pain is one of the common clinical symptoms,which not only has a high incidence,but also is easy to relapse.Usually,patients can feel pain,numbness and fatigue of the waist,mainly for pain and discomfort on one side or both sides.It is generally a localized abnormal sensation,but in severe cases,in addition to waist discomfort,there will also be radioactive pain in the lower extremities.As a common clinical symptom,low back pain not only appears in pain department,acupuncture department and orthopaedics department,but also causes waist discomfort in other departments,especially in gynecology and urology.It is also one of the most common reasons for low back pain in clinic."Weizhong" acupoint is the acupoint on the Bladder meridian of foot-taiyang,and the routes of meridians and tendons pass through the low back of the human body.In clinic,"Weizhong" acupoint is selected to treat low back pain,which embodies the principle of"meridian and main treatment".And the "Weizhong" acupoint is the lower He-sea point of the bladder meridian,and the bladder meridian and the kidney meridian are the exterior and interior of each other,and "waist is the viscera of the kidney".Abnormal function of the bladder meridian will cause abnormal function of the kidney meridian,which will lead to low back pain."Weizhong" acupoint is selected to treat low back pain,which embodies the principle of"combining treatment of internal viscera".The record of "seeking in the committee of the back"in " Song of the Four Command Points" is a summary of the experience of selecting"Weizhong" acupoint for the treatment of low back pain,which fully embodies the wisdom of the ancients.The multifida muscle is located in the deep position on both sides of the spine and attached to the spine.It is one of the important core muscles of the human body.Under normal functional conditions,it can help the spine to complete lateral flexion,flexion,extension and rotation.It is one of the important muscles for the body to maintain normal and flexible lumbar movement,and can maintain the stability of the waist and spine.When the multi-cleft muscle is injured or stimulated by external force,when the functional state is abnormal,the fascia,facet joints,ligaments and other tissues around the spine will be out of balance,and when the injury is serious,there will be obvious pain in the waist,and when the nerve is squeezed,there will be radioactive pain in the lower extremities.It is known that the multifid muscle is closely related to the occurrence of low back pain.Exploring the regeneration state of muscle fibers after the repair of multifid muscle injury can well grasp the repair of skeletal muscle and has important guiding significance for the treatment of low back pain.Transforming growth factor TGF-?1 plays an important role in muscle regeneration and muscle fibrosis,connective tissue growth factor CTGF is its downstream factor.TGF-?1 can promote connective tissue CTGF regeneration,and connective tissue proliferation can directly lead to organ and tissue fibrosis.Previous studies have shown that TGF-?1 can induce muscle fibrosis,promote fibroblast proliferation,and their overexpression.All of them are not conducive to the repair of muscle injury,and in severe cases,it will promote the formation of scar tissue.Vimentin,an important indicator of muscle growth,plays an important role in the early stage of muscle regeneration.It can trigger the acti-vation of muscle satellite cells and promote tissue injury and repair.ObjectiveThe purpose of this study was to investigate the effect of electroacupuncture at"Weizhong" point on the related factors of muscle fiber regeneration and fibrosis inhibition after the injury of psoas multifidus muscle,and to find a proper treatment course for the treatment of low back pain.MethodsFifty-four male 8-week-old SD rats were divided into blank group(n=18),model group(n=18)and electroacupuncture at Weizhong point group(n=18).Each group was randomly divided into 1 d,3d and 7d groups,with 6 rats in each subgroup.The blank group did not receive any treatment,while the electroacupuncture group was treated with electroacupuncture once a day for 20 minutes.The model group was fixed synchronously with the electroacupuncture group,but no treatment measures were taken.The histomorphological changes of multifid muscle of rats in each group were observed by Masson staining,the expression of TGF-?1,CTGF and Vimentin was detected by immunohistochemistry,and the protein expression of TGF-?1 was detected by Western-blot method.Results1.Masson staining results:a large number of red-stained muscle fibers could be observed in the blank 1d,3d and 7d groups,and they were neatly distributed and arranged;a large number of blue-stained collagen fibers could be seen in the model group,and a little red muscle fibers were distributed in the model group;compared with the model group,red-stained muscle fibers could be seen in the electro-acupuncture Weizhong point group,relatively speaking,the collagen fibers stained by blue were less.2.Results of immunohistochemical method:TGF-?1:one day after treatment,compared with the blank group,the difference of TGF-?1 between the model group and the acupuncture group was statistically significant(P<0.01,P<0.01),and compared with the model group,the electroacupuncture at Weizhong point group had a downward trend,and the difference was significant(P<0.01).After 3 days of treatment,the expression in the model group and the electroacupuncture Weizhong point group was higher than that in the blank group(P<0.01,P<0.01),and the expression in the electroacupuncture Weizhong point group was lower than that in the model group(P<0.01).After 7 days of treatment,the expression in the model group and the electroacupuncture Weizhong acupoint group was higher than that in the blank group(P<0.01),and the expression in the electroacupuncture Weizhong acupoint group was less than that in the model group(P<0.01).CTGF:One day after treatment,the expression in the model group was higher than that in the blank group(P<0.01),and that in the electroacupuncture Weizhong point group was higher than that in the blank group(P<0.05),and that in the electroacupuncture Weizhong point group was lower than that in the model group(P<0.05).After 3 days treatment,the expression in the model group was higher than that in the blank group(P<0.01),but there was no significant difference between the electroacupuncture Weizhong group and the blank group(P>0.05).After 7 days of treatment,the expression in the model group and the electroacupuncture Weizhong group was higher than that in the blank group(P<0.01,P<0.01),but the expression in the electroacupuncture Weizhong group was lower than that in the model group(P<0.01).Vimentin:One day after treatment,the expression of the model group and electroacupuncture Weizhong group was higher than that in the blank group(P<0.01,P<0.01),and that in the electroacupuncture Weizhong point group was higher than that in the model group(P<0.01).After 3 days treatment,compared with the blank group,the expression in the model group and electroacupuncture Weizhong acupoint group was higher than that in the model group(p<0.01,P<0.01),and that in the electroacupuncture Weizhong acupoint group was higher than that in the model group(P<0.01).After 7 days of treatment,the expression in the model group and the electroacupuncture Weizhong acupoint group was higher than that in the blank group(P<0.01,P<0.01),and the expression in the electroacupuncture Weizhong acupoint group was higher than that in the model group(P<0.05).3.Results of Western-blot method:TGF-?1:On the first day of treatment,compared with the model group,the expression of electroacupuncture at Weizhong acupoint group decreased,and the difference was statistically significant(P<0.05).After 3 days of treatment,compared with the blank group,the expression of electroacupuncture at Weizhong acupoint group decreased,and the difference was statistically significant(P<0.05).After 7 days of treatment,there was no significant difference among the groups(P>0.05).Conclusion1.Electroacupuncture at "Weizhong" point could reduce the expression of TGF-?1 and CTGF,and increase the expression of Vimentin,which is one of the mechanisms to promote the repair of skeletal muscle injury.2.Electroacupuncture can improve the ability of muscle fiber regeneration by inhibiting the expression of fibrogenic factors,so as to achieve the purpose of treating the damage and repair of skeletal muscle in rats.The most obvious effect may appear on the third day.
