The Diagnosis Of "Zhai Nai" Disease And The Clinical Efficacy Study Of Tibetan Medicine "Liyan Buccal"

Research objectives: 1.For a better understanding and the ways of diagnosis of "Drenad" disease based on Tibetan Medicine's literatures.2.For collecting and sorting out the therapeutic agents for the treatment of "Drenad" disease in the classical literature of Tibetan medicine,and excavating the commonly used drugs and treatment rules for the "Drenad" disease.3.According to the medication rules of Drenad disease,a prescription for the treatment of Drenad disease was selected to develop a buccal tablet for relieving sore throat.4.To observe the changes in the scores of symptom scales of Tibetan medicine before and after the treatment of "Drenad" disease with this buccal tablet,and to objectively evaluate its clinical efficacy.Methods: 1.Diagnosis: by collecting and organizing etiology,clinical manifestation,classification of Drenad disease based on the largest Tibetan literature book The Classical Tibetan Medicin Book,inducted the causes,clinical symptoms,classification of this disease,and compared this disease to other four disease Badcan gulgak,Kadgak,Drecham,Kakpa to differentiate the differences and similarities between them rely on the literature,comprehensively analyzed the method of diagnozing Dernad disease by Tibetan Medicine.2.Medication law analysis: Collect the largest Tibetan literature books to find the treatment of Drenad disease and drug prescriptions,and using SPSS Statistics,SPSS Modeler 18 and 23 Gephi 0.9.2 software such as descriptive Statistics,correlation analysis method,the complex Apriori algorithm network diagram analysis method,combining the theory of "flavor taste" of Tibetan Medicine,build treatment of Tibetan Drenad disease "prescription-medicine-property-effect" of the drug conceptual framework,excavate the law of treaing Drednad disease of Tibetan Medicine,it also analyzes and summarizes the clinical ideas and common prescriptions and drugs of Tibetan medicine for the treatment of Drenad disease.3.Development of the buccal tablet: based on the rule of the Tibetan medicine for the treatment of the Drenad disease,a Tibetan medicine for the treatment of the Drenad disease was screened through a modern drug extraction method(water + Alcohol)developed the first phlogistic buccal tablets in Tibetan medicine.4.Clinical research: Due to the lack of recognized diagnostic standards and curative effect assessment scales,combined with the literature study of Drenad disease,screened the main symptoms,and drawnup the diagnosis and curative effect evaluation plan.And learning from instructors and relevant clinical experts to demonstrate " The diagnostic criteria and curative effect assessment scale of Drenad disease,so that 33 patients were included with this standard to observe the change of symptom scores before and after treatment to assess their clinical curative effect.Research results: 1.Research on Tibetan medicine literature of Drenad disease: The study found that a total of 36 classic Tibetan medicine documents recorded 259 symptoms and diagnostic factors related to Drenad disease,and there are 57 symptoms and diagnostic factors after repeated elimination,of which frequency Symptoms with a ratio greater than 10% are swollen throat(22),thick phlegm(15),pharyngeal congestion(12),throat pain(12),throat burning sensation(10),throat dryness(9),viscous secretions(7),dry mouth(6),ear pain(7),pharyngeal foreign body sensation(6),pharyngeal rash or papules(7),there are eleven clinical manifestations in total.The syndrome classification of Drenad disease in the Tibetan medical literature can be summarized into two categories: "three causes" classification and main symptoms classification.“three causes” include Lung type((?)),Chrakchri type((?)),badcan-type((?)),integration type((?)).main symptoms classification include Dras type((?)),Manbu type((?)),Tsakam((?)),Tsagak((?)),Dregak((?))etc.Literature studies have found that the pharyngeal "Chrak" is strong,so Drenad is mostly tend to hot nature.After analysis with Badcan gulgak,Kadgak,Drecham,Kakpa,it was found that the Drenad disease has the same etiology and clinical symptoms as Kadgak,and they belonging to different classifications of different parts and spreads within the same disease.Kadgak sounds are mostly due to diseases affecting the vocal cords.The clinical manifestation of hoarseness is the main symptom.Drenad disease does not affect the vocal cords,so there is no symptom of hoarseness.The remaining three diseases and drenad are different diseases at the same site,and the causes and symptoms are very different.2.Medication rule of Drenad disease: Collected and excavated literature found a total of 223 prescriptions of Tibetan medicine for treating Drenad disease,which consisted of 168 drugs,the cumulative frequency of single drugs reached 1406 times,and the average number of flavors of the prescription was 6.3,Myrobalan,alpine gentian flower,licorice,geranium,clove and other drugs are mainly sweet,bitter,and mainly cold,heavy,dull,soft and other medicinal properties in terms of the seventeen effects.3.Prescriptions: Based on the medication rules and treatment ideas of the Drenad disease,the treatment theory of Tibetan medicine for Drenad disease is the dominant method to screen the "Thirteen Artemisiaareraria" which is a better representative prescription for the treatment of Drenad disease.According to the Tibetan medicine plus and minus formula theory,with the same proportion of Myrobalan and a small proportion(1000: 1)Musk constitutes "buccal tablets (?)".After crushing the raw materials of the prepared prescription,8905 grams of the original drug powder are obtained,and 1010 grams of extract are extracted by a modern drug extraction method(water + alcohol extraction)with an extraction rate of 11%.According to the calculation of taking 2 grams of raw material medicine,0.22 grams of extract medicine is taken at a time,and tablets are made with this number of grams.Based on the results of literature research,combined with clinical manifestations,the pharyngeal congestion,pharyngeal pain,pharyngeal swelling,thick phlegm,pharyngeal burning sensation,dry throat sensation,thick secretions,dry mouth,ear pain,pharyngeal foreign body such as sensation,pharyngeal rash or papule,these eleven clinical manifestations are the main observation objects,and it is planned to formulate the diagnostic criteria and evaluation criteria for the efficacy of Drenad disease.Enrolled 33 cases of Drenad patients,treated for 7 days according to the treatment standard,the results of clinical efficacy evaluation were cured 33.3%(11 people),markedly effective 30.3%(10 people),effective 30.3%(10 people),ineffective 6%(2 People),there was a significant difference in the scores of symptoms before and after treatment(P <0.01).Therefore,the Tibetan medicine buccal tablet for drenad has many clinical application values for the treatment of Drenad disease,with obvious curative effect,no side effects,simple operation and low price.Conclusion: 1.Drenad is caused by blood,so Drenad is mostly hot.2.The treatment of Drenad disease by Tibetan medicine is mainly based on Jinping Chripa,supplemented by Anqi Ninglong.The tastes are mainly sweet and bitter drugs.The natures are are mainly cold,heavy,dull,soft and other medicinal properties.3.Combined with the theory of Tibetan Medicine and modern tablet technology,we successfully developed the first Tibetan tablet "buccal tablet" for throat based on Tibetan medicine.4.This tablet have the characteristics of obvious curative effect and no side effects,and have very high clinical application value and broad development prospects.
