Study On The Diagnostic Criteria Of The Syndrome Of Phlegm And Blood Stasis In Coronary Heart Disease And Angina Pectoris

Objective: To establish a standardized diagnostic Standard of phlegm and blood stasis syndrome in angina Pectoris of Coronary Heart Disease.Methods:1.To study the diagnostic efficacy of the diagnostic criteria drawned up by previous literature studies and expert questionnaires,evaluate the diagnostic efficacy by using consistency analysis,sensitivity,specificity,and so on.In terms of reliability and authenticity,the efficacy of TCM diagnostic scale for phlegm and blood stasis syndromes of coronary heart disease was comprehensively evaluated,and its clinical application value was evaluated.2.Through analysing four diagnosis items of all 150 cases,screening and then assigned of valuable four diagnostic items,exerting the discriminant coefficient method,established the quantitative standard of phlegm and blood stasis syndromes of angina pectoris of coronary heart disease.3.The consistency analysis of the new quantitative standard and reference standard was carried out,and the diagnostic effectiveness of the new standard is compared with that of the previous standard.Results: 1.The Kappa value of consistency test between diagnostic standard and reference standard,sensitivity,specificity,missed diagnosis rate,misdiagnosis rate,accuracy,positive likelihood ratio,negative likelihood ratio was 0.708,78.21,93.06,21.79,6.94,85.33,11.26 and 0.23,respectively.There is a high degree of consistency between the breaking standard and the reference standard.It has high diagnostic value for phlegm and blood stasis syndrome of coronary heart disease.2.Quantitative criteria based on maximum likelihood method: chest pain × 4 + body weight × 3 + mouth sticky × 4 + lips purple × 6 + ruffian full × 3 + stool sticky × 7 + tongue purple dark × 5 + sublingual purple × 6 + fur × 6 + Pulse acerbity × 5 + pulse string slippery × 4 + pulse string acerbity × 5 + chest pain position fixed × 6 + chest pain at night accentuated × 7 + lips or gingival color dark × 5 + skin nail × 9 + complexion black × 9.3.Compared with the experimental diagnostic criteria developed earlier,the RRR is 0.125.Compared with the previous test diagnostic criteria,It is more consistent with the reference standard and has high diagnostic value for phlegm and blood stasis syndrome of coronary heart disease.Conclusion:Tested the Diagnostic criteria for angina Pectoris with phlegm and Blood stasis in Coronary Heart Disease drawn up by literature studies and expert questionnaires through clinical studies;Furthermore,established the quantitative diagnostic criteria of phlegm and blood stasis syndrome in angina pectoris of coronary heart disease.
