Study On The Correlation Between Heart Failure And Frailty And Serum Cholinesterase In The Elderly

Purpose: Statistically analyze the correlation between heart failure(HF)patients and debilitating and serum cholinesterase,and explore the different TCM syndromes and serum cholinesterase value,NT-probnp value,LVEF value and heart function The rules of change between grades and so on provide more basis for judging the prognosis of patients with heart failure and the classification of TCM syndromes.Material and method: Cases originated from January 2019 to March 2020 in the Department of Cardiovascular Medicine and Aging and Senile Wards of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine and were diagnosed with heart failure.140 cases were selected based on the inclusion and exclusion criteria.For patients with heart failure who signed the informed consent,collect and organize patient data,including recording the patient's name,age,gender,height,weight;clinical symptoms and signs of the patient,diagnosis and diagnosis of the condition,diagnosis of weakness,dialectical classification of TCM and heart function Classification,color Doppler echocardiographic examination results,laboratory examination results,and preparation of clinical questionnaires for patients with heart failure and Tilburg weakness assessment scale,the establishment of a database,divided into two groups according to the degree of weakness;the cases were divided according to the type of TCM syndromes Five groups;statistical analysis of relevant data.Results:There is no significant difference in gender distribution between the heart failure without weakness group and the heart failure with weakness group(P> 0.05);there is a difference in the NT-pro BNP value distribution(P <0.05);there is no significant difference in serum cholinesterase value Differences;meanwhile,the two groups had significant differences in heart rate,blood pressure,alcohol consumption,hypertension,coronary heart disease,diabetes,prealbumin,triglycerides,cholesterol,high density lipoprotein,low densitylipoprotein,and LVEF.The difference(P <0.05),the patients with heart failure and no weakness were generally less drinking than the heart failure with weakness group,and heart rate,blood pressure,prealbumin,triglyceride,cholesterol,high density lipoprotein,low The density lipoprotein and LVEF values were better than those in the weak group;however,there was no significant difference in blood glucose values and smoking status between the two groups(P> 0.05).However,in comparison of the values,the values in the heart failure with weak group were still higher than those in the heart failure group.Accompanied by debilitating group.Deficiency or not is related to TCM syndrome types.Patients with heart failure without weakness are mainly concentrated in Qi deficiency and blood stasis syndrome(94.0%),without Qi deficiency and yin deficiency syndrome and Yang deficiency water flooding syndrome;patients with heart failure and weakness group Four types of syndromes are covered.The highest proportion is the syndrome of deficiency of qi and yin(34.4%),followed by the syndrome of deficiency of yang and water(26.7%).In the analysis of the correlation between TCM syndrome types and lvef,NT-Probnp,cholinesterase,and cardiac function classification,the TCM syndrome types were negatively related to lvef,cholinesterase levels,and positively correlated with NT-Probnp and cardiac function classification..At the same time,the correlation between weakness and lvef,NT-Probnp,and cholinesterase grading was analyzed,and the results showed that: weakness was negatively correlated with lvef,cholinesterase,and positively correlated with NT-Probnp value.Conclusion:1.Through the analysis of baseline data,it is found that patients with heart failure and weakness have differences in past history and life history,and there is a history of drinking,and heart failure patients with previous basic diseases are more likely to be accompanied by debilitating conditions.2.According to the proportion of TCM syndrome types of patients in different groups,patients with heart failure without weakness are commonly seen in the clinical syndromes of qi deficiency and blood stasis,and most of them are mixed with deficiency and solidity.The patients with asthenia group mainly have two syndromes of deficiency of qi and yin andgeneral syndrome of deficiency of yang and water.The deficiency is the main syndrome.Clinically,they should be strictly dialectical.3.Serum cholinesterase is negatively correlated with TCM syndromes and degeneration.It can be used as one of the indicators for clinical evaluation of patients' debilitating conditions based on the level of cholinesterase levels.According to the patient's clinical manifestations and other indicators,such as LVEF,NT-pro BNP or other biochemical indicators.
