Objective: Through the collection of the medical records of the patients with white fever in the Affiliated Hospital of Liaoning University of traditional Chinese medicine, the characteristics and clinical effects of early-onset(EOP)and late-onset(LOP)white fever were analyzed and compared, and the similarities and differences between the patients with early-onset(EOP)and late-onset(LOP)in laboratory examination, complications and clinical treatment were discussed,so as to accumulate data for the further study of psoriasis.Data and Methods: We collected the patients with white fever of common type admitted to dermatology ward of Affiliated Hospital of Liaoning University of traditional Chinese medicine from April 1,2017 to March 31,2019.Establish the clinical data database through Excel software and record the patient information in detail, mainly including: general information(name, gender, age of treatment, course of disease, height, weight, season of onset, family history, smoking and drinking history);clinical information(past history, inducing and aggravating factors, location of onset, laboratory examination, tongue image and pulse image);treatment(traditional Chinese medicine treatment, western medicine treatment)Medical treatment,PASI score before and after treatment);and the patients were followed up by telephone to record the recurrence of skin lesions within 6 months after discharge.Result: 1.General situation: This study included 81 cases of white haze patients with blood heat syndrome, including 62 males and 19 females, with a male to female ratio of 3.26:1.There were 44 cases in EOP group,30 males and 14 females. There were 37 cases in Lop group,32 males and 5 females. There was no significant difference in gender composition between EOP group and Lop group(P > 0.05);the maximum age of the patients was 83 years old and the minimum age was 18 years old, with an average age of 51.60 ± 14.66 years. The average age of patients in EOP group was 44.52 ± 15.04 years old,and that in Lop group was 60.02 ± 8.42 years old.The age of patients in Lop group was significantly higher than that in EOP group(P.The duration of EOP group was significantly longer than that of Lop group(P < 0.05),and there was a significant negative correlation between the age of the first onset and the duration of the disease(correlation coefficient was-0.498,P < 0.01);there was no significant difference in the onset season,BMI,smoking and drinking history between the two groups(P > 0.05);there was no significant difference in family history between the two groups(P > 0.05),but the age of the first onset of the patients with positive family history was significantly younger than that of the patients with positive family history Patients with negative family history(P < 0.05).2.Medical records: compared with EOP group, the patients in Lop group were more likely to have diabetes, hypertension and cerebral infarction(P.The tongue texture of 81 patients was mainly red tongue(80 cases),tongue coating was mainly yellow fur(50 cases),pulse condition was mainly stringed smooth pulse(26 cases)and smooth pulse(29 cases).3.Treatment and medication: through descriptive analysis of 81 patients' medication frequency of traditional Chinese medicine decoction, the top 14 herbs from high to low were: Shengdi, shenglicorice, red peony,Danpi,Hedyotis diffusa,Baicao root,Caulis Spatholobi, honeysuckle,Sophora japonica,Zicao, forsythia,Wumei,Banzhilian and Danshen. There was no significant difference in the use of traditional Chinese medicine between the two groups, and there was no significant difference in the use of other traditional Chinese medicine and Western medicine treatment between the two groups(P > 0.05).4.Curative effect: the PASI score of 81 patients after treatment was significantly lower than that before treatment(P < 0.05),but there was no significant difference between the two groups(P > 0.05).81 patients were treated in hospital, the curative effect was 0,62,19 and 0,the total effective rate was 76.54%.There was no significant difference between the two groups in the therapeutic effect and the recurrence of skin lesions within 6 months after discharge(P > 0.05).Conclusion:In this study, the patients with blood heat syndrome were divided into two groups1.The younger the age of first onset is,the longer the course of disease is;the younger the age of first onset is.2.Lop patients are more likely to be associated with type 2 diabetes, hypertension and cerebral infarction;the older the first onset, the higher the fasting blood glucose and urea levels.3.EOP and Lop patients are similar in the treatment of traditional Chinese medicine and Western medicine. They have good curative effect on both EOP and Lop patients. The total effective rate is 76.54%. |
