Objective :.Through retrospective analysis,the clinical effect of tension-pull skin extender and traditional method for closed incision and decompression of lower limb bone fascia was compared,and the clinical Therapeutic Scheme and reference for safe and effective closed wound were provided by comparative study.Methods: The patients who were treated by incision and decompression in the orthopaedic ward of The First Affiliated Hospital Dalian University Medicial in July2014 ~ December 2017,were enrolled in 14 patients.According to the different methods of incision closure after decompression,it was divided into skin extender treatment group and traditional treatment group.The Skin Extension treatment group: 3 cases of male patients,2 cases of female patients,age 20-64 years old,the average age of 43.8±16.7 years old,and its decompression after the incision are high tension incision;traditional methods of treatment group: 5 cases of male patients,4 cases of female patients,age 10-78 years,the average age was 46.8±21.8,and decompression was formed into a high tension incision.Using the retrospective analysis,the two groups of patients with age,sex,wound formation time and other general conditions,the operation time,postoperative antibiotic use days,the number of postoperative dressing,the length of the hospital after incision,incision healing scores,etc.All the data in this study were analysized by SPSS 22.0,the measurement data were expressed by (?)±s,comparing the difference between the two groups of our two independent samples according to using T-Test,and comparing the counting data with Fisher Precision Test.If p<0.05,the previous differences were thought to be statistically significant.Results: The data from the two groups of patients were statistically compared,the sex,age,the time of decompression and the wound period(i.e.the wound formation time)were analyzed and compared.As p>0.05,the differences which described previously were not statistically significant,so the two groups had no statistical significance in the operation time,the antibiotic use days after our operation,The number of dressing change,the number of hospitalization days after incision and the score of wound healing,as p< 0.05,the difference was statistically significant.The skin extension group and the unused skin extension group were not used during the operation time with the skin extension group less time-consuming than the skin extension group,as our result shows p< 0.05,so the difference was statistically significant,as compared with the number of days after operation of antibiotics,the number of dressings after operation,the number of hospitalization days after the incision of fascial room,and so on,the skin extension group was less than the p<0.05,so the difference was statistically significant.In the incision healing score,The skin extension Group was also superior to the p<0.05 skin Extension group,so the differences previous described were statistically significant.Conclusion: The use of a lever-type skin Extender enables the effective use of the skin's viscoelasticity,full stretching of the lower limb fascia compartment syndrome incision and decompression of the skin on both sides of the incision,so as to obtain more additional skin,so that the lower limb bone fascia compartment syndrome decompression after the incision under the low tension closure of the ideal to be achieved,Thus reducing the traditional treatment methods of skin graft rate,as well as the two-stage skin graft surgery associated with complications and surgical risks,reducing the patient's hospitalization costs,hospitalization time is significantly shortened,but also reduce the patient's many surgical treatment pain.The treatment technique has the advantages of convenient operation procedure,small tissue trauma and obvious therapeutic effect,and it provides more safe and high therapeutic measures for the incision after decompression of the closed fascia compartment syndrome.
