Correlation Analysis Of Related Factors And Blood Stasis Syndrome Influencing The Early Curative Effect Of PVP Operation In The Treatment Of Osteoporotic Vertebral Compression Fractures

OBJECTIVE:To explore the related factors that affect the postoperative efficacy of percutaneous vertebroplasty(PVP)in the treatment of osteoporotic vertebral compression fracture(OVCF),to judge the prognosis and guide the clinical practice.METHODS:Retrospective analysis January 2020,2017 Patients with osteoporotic vertebral compression fracture and PVP were treated in the Second Affiliated Hospital of Hunan University of traditional Chinese medicine in January,2001.The preoperative,intraoperative and postoperative factors(bone density,blood stasis syndrome score,gender,age,whether to use traditional Chinese medicine or not,compression rate of anterior edge of injured vertebra,number of fracture vertebrae,injection amount of bone cement and dispersion degree of bone cement)were recorded After the first day and the first month after the operation,the efficacy indexes(modified macnab evaluation standard)were recorded,and then the single factor analysis(rank sum test)was carried out to exclude some unrelated influencing factors(the factors with P value of 0.05 or higher were still recorded),and then the remaining influencing factors were analyzed by multifactor(ordered)logistic regression analysis to get the correlation between these influencing factors and the efficacy Sex.RESULTS:Through the statistical analysis of the first day after operation: through the single factor(nonparametric rank sum test),we found that the P value of gender(P=0.665),the number of fractured vertebrae(P=0.845),the amount of bone cement injected(P=0.197)was greater than 0.05,and there was no significant statistical significance.Age(P=0.000),whether to use traditional Chinese medicine(P=0.032),bone density(P<0.001),blood stasis syndrome score(P<0.001),and before injury of vertebrae There was statistical significance in the results of marginal compression rate(P=0.001),diffusion degree of bone cement(P=0.056),and whether bone cement exudates(P=0.012).Then,the factors with statistical significance were analyzed by logistic regression analysis.The results showed that: through parallel line test(P=0.908),the logistic regression of multiple factors(order)was feasible,and the score of blood stasis syndrome was p=0.000,B=-0.302,OR=0.739(0.626-0.872),BMD:P=0.000,B=0.730,OR=2.075(1.559-2.762),age:P=0.000,B=-0.101,OR=0.904(0.869-0.940);One month after operation,statistical analysis was carried out: the p value of gender(P=0.553),bone cement injection amount(P=0.538)and fracture vertebral body number(P=0.289)was greater than 0.05 by single factor(rank sum test).There was no significant difference in age(P<0.001),blood stasis syndrome score(P<0.001),bone density(P<0.001),compression rate of the anterior edge of the injured vertebral body(P=0.001),and whether the bone cement oozed out(P=0.010),whether to use traditional Chinese medicine(P<0.001),bone cement dispersion degree(P=0.032)were statistically significant.Then,the factors with statistical significance were analyzed by multifactor(ordered)logistic regression analysis.The results showed that: through parallel line test(P=0.120),the multi factor(ordered)logistic regression was feasible,including age: P<0.001,B=-0.109,OR= 0.897(0.857-0.939),BMD: P < 0.001,B =0.635,OR=1.887(1.457-2.443).Conclusion: The effect of percutaneous vertebroplasty on the treatment of osteoporotic vertebral compression fracture is affected by many factors.In the short-term effect,it is more closely related to age,bone density and the score of blood stasis syndrome.It is negatively related to age and the score of blood stasis syndrome.That is to say,the lower the age and the score of blood stasis syndrome is,the more obvious the effect is.The greater the bone density is,the better the effect is.At one month,because of the mechanism and absorption of blood stasis,curative effect and age.
