Observation Of The Curative Effect Of Compound Dachaihu Decoction For Postoperative Obstructive Jaundice (liver And Gallbladder Damp-heat Syndrome)

Objective:To observe the clinical effect of compound Dachaihu Decoction in the treatment of obstructive jaundice caused by cholelithiasis,and to explore the mechanism of compound Dachaihu Decoction in the treatment of obstructive jaundice.Methods:From January 2018 to December 2019,40 patients who met the inclusion and exclusion criteria were selected from the First Affiliated Hospital of Hunan University of traditional Chinese medicine.The treatment group(n=20)and the control group(n=20)were divided into two groups.The control group was treated with corresponding operation,anti infection,liver protection and fluid supplement;the treatment group was treated with compound Dachaihu Decoction(vinegar Chaihu 15 g,Zhishi10g,Scutellaria 10 g,Yujin 10 g,Jinqiancao 15 g,paeony 10 g,ginger 10 g,rhubarb 5g,fabianxia 10 g,Yinchen 15 g,gardenia 10 g,and jujube 3 pieces)on the basis of control,and took one dose of 200 ml daily for 7 days.(1)The changes of liver function(DBIL,TBIL,ALT,AST,ALP)and inflammatory index(WBC)were observed.(2)Observe the change of TCM clinical syndrome before and after treatment.(3)Observe the comparison of comprehensive effect before and after treatment.The data were processed by spss26.0 software system.Results:1.General:There was no significant difference between the two groups in age,gender and location of cholelithiasis(P> 0.05).2.Comprehensive curative effect:After statistical analysis,the comprehensive curative effect of the two groups after OJ was compared,z =-2.241,the difference was statistically significant(P< 0.05).3.TCM syndrome score:After statistical analysis,the differences of TCM Syndromes of OJ patients in the two groups before treatment werestatistically significant(P < 0.05)and comparable.After treatment,compared with before treatment,there were significant differences(P <0.01)in the treatment group except for "short red urination" and "constipation" in the control group;after treatment,compared with the control group,there were statistically significant differences(P < 0.05)in "yellow face","full stomach" and "appetite" in the treatment group.There was no statistical significance(P > 0.05)between the two groups of OJ patients before treatment,which was comparable;after treatment,there was a difference between the two groups of OJ patients after operation(P< 0.01);after treatment,the difference between the two groups was statistically significant(P< 0.05).4.Liver function and inflammatory index:Before treatment,there was no significant difference in liver function(DBIL,TBIL,alt,AST,ALP)and inflammatory index(WBC count)between the two groups(P> 0.05),which was comparable;after treatment,there was significant difference compared with before treatment(P < 0.01);after treatment,there was significant difference compared with the control group(P< 0.05).Conclusion:Compound Dachaihu decoction can effectively improve the syndrome of dampness and heat of liver and gallbladder after operation of obstructive jaundice caused by cholelithiasis.To some extent,it can reduce the damage of hepatocytes,accelerate the recovery of liver function,and help to eliminate the inflammation of biliary tract.The prospect of the perioperative treatment of cholelithiasis with the combination of Chinese and Western medicine is gratifying,which is worthy of systematic and in-depth exploration by the majority of medical workers.
