In order to guarantee the basic medication needs of people in countries with low drug production capacity and limited medical and health resources,the World Health Organization(WHO)put forward the concept of essential medicines in 1977 and popularized them widely to ensure that people enjoy basic medicines for health care.In April 1979,the former Ministry of Health and the former General Administration of Medicine were organized to set up a "National Essential Drugs Selection Group" to select and compile a list of essential drugs and formulate policies related to essential drugs according to the actual situation in China.In 2007,the report of the Seventeenth National Congress of the Communist Party of China emphasized that "we should build a drug supply security system covering urban and rural residents,establish a national essential drug system,and ensure the basic drug use of the broad masses of the people".This shows that the state has promoted the formulation of the essential drug system to a strategic level.In 2009,the State Council issued "Opinions on Deepening the Reform of Medical and Health System" and "Opinions on the Implementation of Establishing a National Essential Drug System",which symbolized the formal implementation of China's Essential Drug System.Through the collation and in-depth study of the relevant literature,combined with the existing data and field survey results,this paper analyzed the operation of grass-roots medical and health institutions in Sujiatun District of Shenyang City,the awareness and satisfaction of urban and rural residents and grass-roots medical personnel on the basic drug system,and the financial subsidies of the government to grass-roots medical and health institutions.To understand the implementation of the essential drug system in Sujiatun District,explore the main factors affecting the implementation of the essential drug system,and put forward some suggestions for the better implementation of the national essential drug system in primary medical and health institutions.By using the methods of literature research,questionnaire survey and interview,taking the primary medical and health institutions in Sujiatun District of Shenyang City as the research object,this paper investigates the principals of primary medical and health institutions,medical staff and urban and rural residents of primary medical and healthinstitutions in Sujiatun District,and analyses the survey data.To find out the problems existing in the implementation of essential drug system in primary medical and health institutions in Sujiatun District.The main effects of implementing the essential drug system in primary medical and health institutions in Sujiatun District are as follows: the total number of outpatients and emergency patients in primary medical and health institutions has increased,but the number of hospitalized patients has decreased;the medication behavior has gradually become rational;and the proportion of government financial compensation has increased year by year.The main problems in the implementation of the essential drug system are: the publicity of the essential drug system needs to be improved,the drugs in the essential drug catalogue can not fully meet the clinical needs,the supply of essential drugs can not be fully guaranteed,and the compensation mechanism and incentive mechanism of grass-roots medical and health institutions are not perfect.Based on the field investigation and analysis of the essential drug system in primary medical and health institutions in Sujiatun District,in view of the existing problems in its implementation,it is suggested that the publicity of the essential drug system should be strengthened and the essential drugs should be selected scientifically so as to meet the basic medical needs of the people more comprehensively,and the guarantee system for the production and supply of essential drugs should be established and perfected.We should improve the compensation mechanism of the government and the staff incentive mechanism of grass-roots medical and health institutions step by step.
