Study On The Correlation Between Different TCM Syndromes And Insulin Resistance In Patients With Type 2 Diabetes With Insomnia

Purpose:To explore the correlation between different TCM syndromes and insulin resistance in type 2 diabetes mellitus with insomnia,and to provide scientific basis for clinical diagnosis and treatment.Materials and methods:180 patients with type 2 diabetes mellitus accompanied by insomnia were investigated by questionnaires,and the Pittsburgh Sleep Quality Index(PSQI)was completed to assist the patients to complete the Pittsburgh Sleep Quality Index(PSQI)and to diagnose TCM syndromes.The characteristics of different TCM syndromes were analyzed.Fasting insulin(FINS),fasting blood glucose(FPG),glycated hemoglobin(HbA1c)and total gallbladder were recorded for all the selected patients.Sterol(TC)and triglyceride(TG)were used to calculate insulin resistance index,and the results were analyzed statistically.Results:1.The main syndromes of type 2 diabetes mellitus with insomnia were Yin deficiency and fire exuberance(35.56%),the others were phlegm-heat internal disturbance(20.00%),liver depression and fire(17.78%),heart-spleen deficiency(15.55%)and heart deficiency and timidity(11.11%)with statistical difference(P < 0.05).2.There were significant differences in HOMA-IR among the five TCM syndromes of type 2diabetes mellitus with insomnia(P < 0.05),among which the insulin resistance index(HOMA-IR)of the patients with deficiency of both heart and spleen was the highest and that of the patients with stagnation of liver and fire was the lowest.The HOMA-IR of deficiency of both heart and spleen was significantly different from that of Yin deficiency and fire exuberance syndrome and stagnation of liver and fire syndrome(P < 0.05).3.The insulin resistance index of type 2 diabetes mellitus with insomnia was different with different PSQI scores(P < 0.05).4.The P SQI score of type 2 diabetes mellitus with insomnia was positively correlated with WC,BMI,HbA1 c,TG and TC(P < 0.05),but not with systolic and diastolic blood pressure(P > 0.05).Conclusion:1.The degree of insulin resistance in different TCM syndromes of type 2 diabetes mellitus with insomnia is different.2.The severity of sleep disorder in type 2 diabetes mellitus with insomnia is correlated with the degree of insulin resistance.3.Sleep quality in type 2 diabetic patients with insomnia was significantly correlated with waist circumference(WC),body mass index(BMI),glycosylated hemoglobin(HbA1c),triglyceride(TG)and total cholesterol(TC).
