Background:Sleep disorder is a common health problem.Insomniacs often complain about difficulty falling asleep,difficulty sleeping,waking early,etc.And these sleep problems caused daytime dysfunction in patients.The quality of life is reduced and social function is obviously impaired.Sleep disorder is often accompanied by mood disorders such as depression and anxiety,as well as the progression of physical diseases such as high blood pressure and coronary heart disease.Its effect on the brain is mainly cognitive impairment,especially the decline of memory function.The hippocampus is closely related to memory function,which is the organ that plays a major role in the encoding and extraction of human memory function.There was a significant correlation between the size of the hippocampus and the structural damage of white matter and memory loss.The hippocampus is also a structure associated with sleep.Image for sleep research mainly concentrated in the resting state means such as magnetic resonance imaging(MRI)and magnetic resonance spectrum,including functional connection,the hippocampus morphology,these results also many inconsistencies,seldom using diffusion tensor imaging techniques to understand the brain white matter damage in patients with insomnia disorder.objective:To explore the relationship between insomnia disorder and hippocampal structure,functional imaging and memory function.Methods:According to the diagnostic criteria of DSM.V,a total of 22 cases of patients with insomnia were collected,and the control group was a healthy sleeper with 17 cases,mainly from the staff of the hospital and their healthy relatives.Collect general information such as gender,age and education level.The Pittsburgh sleep quality index(PSQI)and the Epworth sleep quality scale were used in the case group and control group.(Ess)to assess the subjects' subjective Sleep Quality,Insomnia Severity Index(ISI)Severity evaluation of Insomnia,Hamilton depression scale-17(HAMD-17)determine the patient's level of Depression and Hamilton anxiety scale(HAMA)determine the patient's level of Anxiety,Montreal Cognitive Assessment scale(MoCA)a review of the testers of memory,nine-boxmaze test to assess memory.Each subject underwent brain magnetic resonance imaging(MRI)to calculate the standard hippocampal volume.at the same time,use Diffusion Tensor Imaging(DTI)technique,The workstation automatically generates scores anisotropy(FA)and apparent diffusion coefficient(ADC),FA and ADC values of the subjects are calculated respectively.Conclusion:1.Compared with the control group,there was significant difference in ISI,PSQI,HAMDand HAMA scores(P<0.05),but there was no significant difference in ESS,gender,age,and Mo CA scores(P>0.05)between the two groups.2.The insomnia disorder group was compared with the control group in object working memory and spatial working memory,the difference was statistically significant(P < 0.05).There is no significant difference in spatial reference memory,the memory of object reference and object recognition(P> 0.05).3.The volume of left,right and total hippocampal in patients with insomnia decreased significantly compared to the control group(P<0.05).4.Negative relationships was observed between standard left and whole hippocampus volume with the number of errors in spatial working memory(P < 0.05).The smaller volume of the left hippocampal and the whole hippocampus,the worse the spatial working memory.5.The average duration of the disease was over 10 years.Positive relationships was observed between the left,right and whole hippocampus(P < 0.05).The longer the patient's duration,the smaller the hippocampus.6.The FA value of patients with insomnia was significantly lower than that in the control group(P<0.05).The ADC value of patients with insomnia was significantly higher than that in the control group(P < 0.05).7.Negative relationships was observed between FA values with spatial reference memory and spatial working memory(P < 0.05).The smaller the FA value,the worse the spatial memory function.Positive relationships was observed between ADC values with spatial working memory(P< 0.05).The larger the ADC value,the worse the spatial memory function.
