A Clinical Study On The Treatment Of Sleep Disorders After Acute Cerebral Infarction Based On The Method Of Clearing Heat And Removing Phlegm

The purposes:The subject observes the changes of some index,like Pittsburgh Sleep quality Index scale,traditional Chinese medicine syndrome score and polysomnographic parameters before treatment and after treatment,in which we treat the patients suffering from Sleep Disorder after Acute Cerebral Infarction by using Huanglian Wendan decoction.The purpose of the trial is to observe the therapeutic effect of Huanglian Wendan decoction on sleep disorder after acute cerebral infarction with phlegm heat disturbance,and then evaluate effectiveness and safety about the treatment of Huanglian Wendan decoction,besides it has the in-depth understanding of studying the characteristics of polysomnography(PSG)in patients with sleep disorder after acute cerebral infarction caused by phlegm-heat disturbanceMethods:The research adopted the method of randomized,blind,controlled trial methods,and the patients with sleep disorder after acute cerebral infarction were classified according to the syndrome differentiation of traditional Chinese medicine(TCM),the phlegm heat disturbance type was selected as the research object.And then the clinical patients in 60 cases with sleep disorder after acute cerebral infarction were randomly divided into two groups,of which 30 cases were included in the observation group(traditional Chinese medicine treatment group)and 30 cases were included in control group(zopiclone group).Experimental group was treated with Huanglian Wendan decoction,two bags a day,and two times in the morning and evening,while the control group were treated with zopiclone tablets(7.5mg/piece orally).The subject respectively took Pittsburgh sleep quality index(PSQI),TCM syndromes score and polysomnography parameters as observation indexes to make comparison and analysis about sleep process and structure,the evaluation form of TCM Syndrome of sleep disorder after acute cerebral infarction was made to evaluate the curative effect.The course of treatment was 4 weeks,each time before treatment and after treatment,fourth weeks each filled out an observation table,and recorded the corresponding situation.60 cases(30 cases in the observation group and 30 cases in the control group)completed the clinical observation.Finally,in order to observe the clinical effect of Huanglian Wendan decoction and study the characteristics of polysomnography in patients with sleep disorder after acute cerebral infarction caused by phlegm-heat disturbance.SPSS 25.0 statistical software was used to conduct cluster analysis and draw conclusions by making self and matched group comparison before and after treatment.Meanwhile,adverse reactions and adverse events occurred during the period were recorded to evaluate the clinical safety of Huanglian Wendan decoction.Results1.During the treatment,60 clinical patients were included in this research,and finally 60 patients finished the clinical observation,0 cases dropped from the observation group and control group,finally the loss rate was 0.2.General data:gender,age,blood pressure of the two groups were statistically analyzed,and found P values were greater than 0.05,which meant no significance.The two group were comparable3.Pittsburgh sleep quality index(PSQI)score comparison:the PSQI scores of the two groups were significantly lower than those before treatment,and the difference is significant(P<0.05).There was no significant difference in the sleep quality,time of sleep,sleep efficiency,sleep disturbance,hypnotic drugs and daytime dysfunction between the two groups(P>0.05)after treatment,which indicated that the clinical effect of sleep time and sleep disorder is similar.4.TCM Syndromes scale score comparison:experimental group and control group after 4 course of the treatment,the TCM syndromes scores of the two groups were all decreased,the comparison between the two groups(P<0.05)there were statistically differences,which indicated that the improvement of clinical symptoms in the two groups was obviously better than that before treatment.There were significant differences between the treatment group and the control group after treatment(P<0.05),which implied the patients of observation group with syndrome of traditional Chinese medicine to improve the situation is better than control group,indicating that the treatment group was improving the symptom of TCM,such as insomnia,annoyed and irritabil ity,dreaminess,phlegm and chest tightness,dizziness,bitter,mouth,sour,tinnitus,forgetful,constipation scores and other symptoms decreased significantly,with statistical significance(P<0.05).5.Comparison of the sleep parameters by PSG:After treatment,there were significant differences between the two groups(P<0.05),which stated clearly that the improvement of clinical symptoms in the two groups was better than that before treatment.The data distribution was subordinated to normal distribution of parameters and described by mean±standard deviation before and after treatment.After independent samples t-test and analysis of the statistics,there were no differences between the treatment group and the control group after treatment(P>0.05).The result of PSG showed that the total sleep time,sleep efficiency,and percentage of fast eye sleep were all improved after treatment.The sleep time,arousal times,sleep latency,percentage of non-rapid eye movement sleep stage1 were significantly decreased.The slow wave sleep,sleep efficiency were statistically prolonged.Conclusions1.Huanglian Wendan decoction has a good effect on treating sleep disorder after phlegm heat disturbance acute cerebral infarction,which can improve the Traditional Chinese Medicine syndrome of clinical patients.2.Huanglian Wendan decoction had no obvious adverse reaction,and there was highly safety.3.Clearing Heat and Resolving phlegm can obviously improve total sleep time,sleep efficiency,and percentage of fast eye sleep,reduce sleep time,arousal times,sleep latency,percentage of non-rapid eye movement sleep stagel,prolong slow wave sleep,sleep efficiency.
