BACKGROUNDMigraine is a complex and common neurological disease,which seriously affects the work and life of patients and imposes a heavy burden on individuals and society.About 25%-38%of migraine patients need preventive treatment,and topiramate,metoprolol and other drugs are often used for preventive treatment.However,treatment with these drugs often leads to a variety of adverse reactions,and each drug has its limitations.As a result.non-drug therapy is attracting more and more attention.In recent years,a large number of studies have shown that acupuncture can effectively treat and prevent the recurrence and chronic progression of migraine,and improve various symptomssuch as reducing the number of migraine attack days.reducing the frequeny and severity og migraine attack.and improving the quality of life.Although acupuncture is widely used to treat migraine.the neurobiological mechanism behind it is not clear,and some studies even suggest that acupuncture is a placebo effect.OBJECTIVEMany of the imaging studies of rs-fMRI in the treatment of migraine by acupuncture only describe the specific changes in brain regions caused by acupuncture.However,the mechanism of the changes of brain function caused by acupuncture is still unclear,and there are few studies on the underlying mechanism of acupuncture in the treatment of migraine.This study intends to explore and interpret the physiological and pathological mechanism of central nerv ous system of brain function changes induced by acupuncture in the treatment of migraine without aura from the perspective of molecular biology,combining with the previous animal experimental results of our team,so as to provide theoretical support for the biological mechanism of acupuncture in treating migraine without aura,and at the sa,me time provide scientific basis for formulating and improving the diagnosis and treatment program of acupuncture in treating migraine without aura.METHODSForty patients w.ith migraine without aura were included in the outpatient department of acupuncture and moxibustion of Beijing hospital of traditional Chinese medicine affiliated to capital medical university.And they were randomly divided into treatment group and control group,with 20 patients in each group.Both the treatment group and the control group received acupuncture for a period of 4 weeks,3 times a week,a total of 12 times.In the treatment g?oup,acupuncture was performed at effective acupoints,and the control group received acupuncture at non-acupoints.The migraine attacks were recorded from week-4 to week 0,and the brain function imaging data were collected at week 0.And the baseline scales,including HIT-6,BDI-?,BAI,MMSE and MoCA were also filled in.Then BDI-? and BAI were evaluated at week ?,2,3 and 4,respectively,and headache diaries were recorded.And at week 4,HIT-6.MMSE,MoCA evaluations and fMRI scans were performed.Finally,fMRI data collected,information of migraine attacks such as pain days,and HIT-6,BDI-?,BAI,MMSE and MoCA were statistically analyzed,and conclusions were drawn.RESULTS1.There was no significant difference between the treatment group and the control group in the baseline period of HIT-6(p>0.05).After treatment,HIT-6 in the treatment group was ignificantly lower than that before treatment(p<0.001),while in the control group,there was no significant difference in HIT-6 after treatment(p>0.05).2.There was no significant difference between the treatment group and the control group in the baseline period of BDI-?(p>0.05).There was also no significant difference in BDI-? between the treatment group and the control group after treatment(p>0.05)3.There was no significant difference between the treatment group and the control group in the baseline period of BAI(p>0.05).There was also no significant difference in BAI between the treatment group and the control group after treatment(p>0.05).4.There was no significant difference between the treatment grou and the control group in the baseline period of MMSE(p>0.05).After treatment,MMSE in the treatment group was signiiicantly increased than that before treatment(p<0.05),while in the control group,there was no significant difference in MMSE after treatment(p>0.05).5.There was no siLgnificant difference between the treatment group and the control group in the baseline period of MoCA(p>0.0:5).Aftcer treatment,MoCA in the treatment group was significantly increased than that before treatment(p<0.05),wile in the control group.there was no significant difference in MoCA after treatment(p>0.05).6.There was no significant difference in headache days between the treatment group and the control group in thebaseline period.The number of headache days after treatment in the treatment group was significantly lower than that before treatment,while there was no significant difference in the number of headache days after treatment compared with that before treatment in the control group.7.After effective acupuncture treatment,the regional homogeneity of the left upper cerebellum,the left lower cerebellum and the cerebellum vermis of migraine patients in the treatment group was enhanced.After acupuncture,compared with the control group,migraine patients in the treatment group showed reduced regional homogeneity in the temporal lobe,right cuneus lobe,left cuneus lobe,right calcarine cortex,left calcarine cortex,right supra-occipital gyrus,left supra-occipital gyrus,right middle occipital gyrus.and left middle occipital gyrus.CONLUSION1.Ater acupuncture treatment,compared with the control group,effective acupuncture treatment can significantly reduce hit-6,and increase MMSE and MoCA.For BAI and BDI-?,neither effective acupuncture therapy nor sham acupuncture therapy had any therapeutic effect.The reason may-be that the area activated by the fixed acupoint combination acupuncture is relatively fixed,so it cannot be activated by multiple targets and cannot fully take into account the various complications of patients.Therefore,later treatment and research can be based on the acupoints of treatment group,further syndrome differentiation plus or minus acupoints,may receive better clinical efficacy and more in-depth research results.2.After effective acupuncture treatment,headache days in the treatment group were significantly reduced,while after sham acupuncture treatment,headache days in the control group were not significantly improved.3.Effective acupuncture treatment can improve the quality of life and work of patients and improve their cognitive ability by enhancing the regional homogeneity of the left upper cerebellum,lower cerebellum and vermis,and reducing the regional homogeneity of bilateral cuneus lobe,temporal lobe,bilateral superior occipital gyrus,bilateral middle occipital gyrus and bilateral calcarine cortex.
