| Background:Gastric cancer is one of the most common malignancies of the digestive tract.Poor prognosis of gastric cancer is mostly related to the high risk of recurrence and metastasis.Transfer of prevention and treatment of gastric cancer recurrence of traditional Chinese medicine has a certain effect,but the doctor of traditional Chinese medicine for stomach cancer susceptible to recur after people's research is not sufficient,not clear the advantage of TCM prevention and treatment of recurrence of gastric cancer metastasis,constitution of traditional Chinese medicine study as a crowd of TCM classification research hot spot in recent years,is likely to clear the stomach postoperative recurrence of vulnerable groups of traditional Chinese medicine and traditional Chinese medicine to put cure gastric cancer recurrence,transfer the advantage of people have known each other for help,to explore the traditional Chinese medicine physique and the correlation between the post-operation relapse and metastasis of gastric cancer and the effect of TCM prevention and treatment of recurrence of gastric cancer metastasis,for more accurate control of traditional Chinese medicine to recurrence of gastric cancer metastasis and prognosis is of great significance.Objective:This topic proposed on the basis of clinical research,combined with the literature reported that the post-operation relapse and metastasis rate of TCM constitution and gastric cancer and different constitution to evaluate quality of life before and after the treatment,aims to explore the traditional Chinese medicine physique and the correlation between the post-operation relapse and metastasis of gastric cancer and the sensitivity of different constitution of traditional Chinese medicine treatment,from the perspective of traditional Chinese medicine physique,the advantages of TCM prevention and treatment of gastric cancer,help Chinese medicine play a more important role in the field of prevention and cure of cancer of the stomach.Methods:A prospective study was designed for postoperative patients with gastric cancer who visited the oncology department of guang 'anmen hospital,Chinese academy of traditional Chinese medicine from September 1,2016 to September 1,2018.The patients were treated with physical judgment and traditional Chinese medicine differential treatment.The total time of traditional Chinese medicine treatment was no less than 6 months.Once every 6 months follow-up,patients with record review results(imaging examination,tumor markers,endoscopic examination),and fill the EORTC QLQ STO-52 life quality scale,statistical methods are used to calculate transfer nine constitution of patients with gastric cancer recurrence rate,9 kinds of traditional Chinese medicine physique transfer rate of recurrence,analysis the correlation of TCM constitution and general information,disease information.The quality of life of patients with 9 kinds of TCM constitutions 1 year and 2 years after surgery was analyzed by comparing before and after surgery,and the reasons were analyzed.Results:A total of 103 patients were collected in this study,including 90 patients who completed 1-year follow-up and 81 patients who completed 2-year follow-up.(1)constitution and basic data:among the 103 patients in this study,73 were male and 30 were female,with a male to female ratio of 2.8:1.The average age of the patients in this study was 59.3 10.9 years old,with the maximum age of 88 years old and the minimum age of 36 years old.The number of patients aged 50-70 years old accounted for 67.9%of the total number.Gastric antrum was the most tumor site,accounting for 57.3%.Adenocarcinoma was the main pathological type,accounting for 95.1%.The degree of differentiation was the lowest,accounting for 59.2%.Lauren type was the most diffuse type,accounting for 42.8%.Stage III tumor staging was the most common,with 61 patients in stage III,accounting for 59.3%.The stage of tumor invasion was mainly T3 and T4,and the stage of lymph node metastasis was mainly N1 and N3.The main surgical methods were laparotomy and laparoscopic radical resection.Postoperatively,89.3%of the patients received chemotherapy.Peaceful mass 11 people,patients with qi deficiency patients,21 people,qi depression patients with mass 10 people,Yang deficiency patients 34 people,11 patients with Yin deficiency is qualitative,hot and humid 11 patients,patients with phlegm wet mass 1 person,2patients with blood stasis quality,great quality,and the patients with 1 person,among them qi depression to see more women,and deficiency of mass,damp and hot quality to see more,qi deficiency,Yang deficiency and damp heat transfer compared with Yin deficiency more seen in patients with stage III.There was no correlation between constitution and other basic data.(2)correlation between constitution and recurrence and metastasis:among the 103 patients enrolled,23 had recurrence and metastasis.The lower the degree of tumor differentiation and the later the stage,the higher the recurrence and metastasis rate.The constitution with recurrence and metastasis includes qi deficiency,qi stagnation,Yang deficiency,Yin deficiency and damp heat.The recurrence and metastasis rate is the highest in qi deficiency and qi deficiency.Among the 21 patients with qi deficiency and qi deficiency,9 patients have recurrence and metastasis,with a recurrence and metastasis rate of 42.9%,and among the 34 patients with Yang deficiency and qi deficiency,10 patients have recurrence and metastasis,with a recurrence and metastasis rate of 30.3%.(3)evaluation of constitution and quality of life:among the 103 patients enrolled,all completed 12-month follow-up,and 81 patients completed 24-month follow-up.The overall health level of qi-stagnation,damp-heat and Yin deficiency was improved the most after receiving TCM constitution differentiation therapy,and the health level of patients with damp-heat deficiency was improved more slowly than that of qi-stagnation and Yin deficiency.The patients with Yang deficiency and qi deficiency had the lowest basic health level before treatment and the lowest overall health level after treatment.Conclusion:In this study,23 of the 103 patients with postoperative gastric cancer had recurrence and metastasis,with a 2-year recurrence and metastasis rate of 24.7%.The constitution types of recurrence and metastasis were qi deficiency and qi stagnation,Yang deficiency and Yang deficiency,Yin deficiency and hot and humid.Among the 21 patients with qi deficiency and qi deficiency,9 patients had recurrence and metastasis,and the highest recurrence and metastasis rate was 42.9%.Among the 34 patients with Yang deficiency and qi deficiency,10 patients had recurrence and metastasis,and the recurrence and metastasis rate was 30.3%.Through the analysis of EORTC QLQ STO-52 quality of life scale,the overall health level of qi stagnation,damp-heat and Yin deficiency was improved the most after the treatment of traditional Chinese medicine,and the health level of patients with damp-heat deficiency was improved more slowly than that of qi stagnation and Yin deficiency.The patients with Yang deficiency and qi deficiency had the lowest basic health level before treatment and the lowest overall health level after treatment. |
