| Background:Recurrent spontaneous abortion(RSA)refers to two or more consecutive spontaneous a bortions between women and their same sexual partners.2-5%of women in childbeari ng period have the possibility of RSA.The risk of abortion increases with the number of consecutive abortions.After two consecutive spontaneous abortions,that is,RSA,t he abortion rate of patients with second pregnancy can be as high as 50%-80%.The i ncidence of RSA is increasing rapidly in China.At present,the national policy of sec ond births has been liberalized to prevent the recurrence of spontaneous abortion,whic h is of great significance to the protection of female reproductive health.At present,t he prevention and treatment of RSA has become a hot and difficult point in the field of reproductive medicine.The prevention and treatment of RSA is a hot and difficult point in reproductive medicine.Modern medicine considers that RSA is caused by co mplex etiology,heredity,anatomy,endocrine and other factors,and 50%of patients h ave unknown etiology.Recent studies have found that abnormal endometrial receptivity is also an important cause of RSA.The thickness of endometrium can reflect endom etrial receptivity.The main abnormality of endometrial receptivity can be manifested i n thin endometrium.At present,clinical use of estrogen and other drugs and surgical methods to increase endometrial thickness,improve endometrial receptivity,but the tre atment effect is not ideal.The treatment of thin endometrium has become the focus a nd difficulty in reproductive field.This disease belongs to the category of "abortion" and "number abortion" in traditional Chinese medicine.The treatment of RSA by traditional Chinese medicine has a long history,obvious characteristic advantages and definite clinical efficacy.Most physicians give traditional Chinese medicine before and after pregnancy to intervene in the treat ment of RSA,and have achieved good clinical efficacy.However,at present,the clini cal observation of RSA by traditional Chinese medicine mostly focuses on endocrine f actors and immune factors,and there is still a lack of research from the perspective o f endometrial receptivity.After many years of clinical practice,the tutor emphasized the importance of pre-preg nancy intervention of traditional Chinese medicine in the treatment of recurrent abortio n,and observed the clinical effect of pre-pregnancy and post-pregnancy intervention of Kidney-Tonifying and pregnancy-assisting method on recurrent abortion with corpus 1 uteum insufficiency.The results showed that the patients received pre-pregnancy interv ention of Kidney-Tonifying and pregnancy-assisting.It can significantly improve the su ccess rate of re-pregnancy and effectively prevent the recurrence of spontaneous aborti on.On the basis of this study,the tutor also found that Chinese herbal compound prescri ption has many targets in the treatment of diseases,.It is believed that women are bloo d-based,endometrium is tangible and refined by sperm blood.So based on the theory of "ample blood makes it easier to contain uterus" in Fu Qing's Principal Gynaecolo gy,on the basis of Kidney-Tonifying and spleen-stomach-tonifying Kidney-Tonifying a nd pregnancy-tonifying prescription,the products of kidney-yin and blood-nourishing ar e enhanced,and the prescription of Kidney-Tonifying and blood-nourishing is formed,which improves the uterus by increasing the thinner thickness From the perspective of endometrial receptivity,to observe the clinical efficacy of traditional Chinese medicin e in treating RSA caused by endometrial thinning,in order to solve the practical clini cal problems,this topic was established.Research purposes:Through the intervention of tonifyin g kidney and nourishing blood prescription before pregnancy to improve the thin endometrium and improve the clinical efficacy of endo metrial receptive treatment for recurrent abortion,we systematically and objectively obs erved,supplemented the lack of previous studies on recurrent abortion caused by thin endometrium in traditional Chinese medicine,and explored effective and practical ideas and methods to solve the current clinical difficulties.Research contents:Part I:Literature reviewSummary 1:Modern medicine overview of thin endometrial recurrent abortion:Throug h literature research,summarize the research progress of modern medicine on the path ogenesis and treatment of recurrent abortion.Summary 2:Overview of TCM research on thin endometrial recurrent abortion:Throu gh literature research,review and summarize the understanding,treatment and research status of TCM on recurrent abortion.Part II:Clinical ResearchResearch methods:By collecting 30 cases of thin endometrium RSA diagnosed by west em medicine and syndrome differentiation of kidney and spleen insufficiency in traditi onal Chinese medicine,all of them were included in the treatment group.Before preg nancy,they were treated with tonifying kidney and nourishing blood prescription(Cusc uta chinensis,Radix Pseudostellariae,fried turtle plate,cooked ground,etc.)for 3 mon ths.Ask the patient to use contraception during the medication.The thickness of endo metrium was examined on the day of ovulation after withdrawal,and the improvement of the thickness of endometrium was observed.If the thickness of endometrium was more than 7 mm,the patient was asked to prepare for pregnancy.After pregnancy,th e patients were treated with Bushen Antai Prescription to protect the fetus until 12 we eks of pregnancy.The pregnancy outcome,the improvement of endometrium before pr egnancy and the degree of threatened abortion symptoms during the second pregnancy were observed and compared.Result:1.Pregnancy success rate:Among 30 patients,22 were pregnancy success to 12 week s of gestation,and 8 were abortion.The pregnancy success rate was 73.3%,and the a bortion rate was 26.7%.The abortion rate of recurrent abortion patients in this study was significantly lower than that in the literature[2,3],with statistical significance.2.After treatment,the thickness of endometrium on ovulation day increased,the type of endometrium and endometrial blood flow improved,and the difference was statistic ally significant before and after treatment(P<0.05).3.After treatment,the symptoms of threatened abortion were significantly alleviated a nd the pregnancy was maintained.There was a significant difference before and after treatment(P<0.05).Conclusion:1.Tonifying kidney and nourishing blood prescription can increase endometrial thickne ss,optimize endometrial morphology,enrich endometrial blood perfusion and improve endometrial receptivity.2.Tonifying kidney and nourishing blood prescription can improve the pregnancy outc ome of RSA patients by improving the thin endometrium.3.Give full play to the advantages of traditional Chinese medicine treatment,explore effective and practical methods,solve clinical problems,fill the gap in this regard. |
